Incident investigation·January 27, 2026

On January 27, 2026, Licensing Program Analyst (LPA) Monique Ayala conducted an unannounced Case Management inspection at the above facility. LPA met with director, Ariana Alonzo who guided LPA on a tour of the facility. LPA observed 19 infants in care with 8 staff members. The purpose of the inspection is to follow up on an incident report that occurred on 01/13/2026 and was reported on 01/14/2026. Incident was reported in a timely manner. The facility had a recent hand, foot and mouth outbreak at the facility. During the inspection LPA observed sanitation supplies, facility roster and LPA obtained relevant documentation. There are no deficiencies being cited as the facility followed their protocols for outbreaks. Exit interview was conducted and a copy of this report was provided to director, Ariana Alonzo. A Notice of Site Visit was provided and must be posted for 30 days.
