On 07/23/2025 at 9:30 a.m. Licensing Program Analyst (LPA) Peter Bishop arrived at the above facility for the purpose of an unannounced Case Management - Incident Inspection. LPA Bishop announced the purpose of the visit and was granted entry into the facility by Director Quiyanna Ramirez. Census was taken there were 66 children present and 81 enrolled. The purpose of this visit was due to a Unusual Incident Report that occurred on Tuesday, 07/22/2025. Director Quiyanna Ramirez reported the incident to Licensing on 07/22/2025 via regional office at 323-981-3350 to the analyst on duty. At the time of inspection LPA Bishop observed 66 children present doing various activities in their classrooms and outside. Director Quiyanna Ramirez provided tour of facility and showed LPA Bishop where the incident occurred in the Porpoise Paradise Room which is an inside play area with steps, two slides and tunnels for the children to crawl. There were 17 children present in the Porpoise Paradise Room and three Staff. Director Quiyanna Ramirez states that there were three staff with children on the day of the incident. One of the Staff Menmbers S1 witnessed the incident and indicated the children were running and C1 fell and tripped and C2 fell on the other student and her arm was broken. Medical attention was required. LPA did not observe any physical plant issues. Based on Interviews and observations LPA did not observe anything that was unsafe in the room. LPA was able to speak with the parent and the interview is recorded on an additional LIC 812. Director Quiyanna Ramirez confirmed that C1 did in fact have to be treated for a broken arm. The injury did not require stitches. There was adequate supervision at the time of the incident. Based on speaking with Director Quiyanna Ramirez this could not have been prevented. Based on today's inspection there will be no deficiencies cited today in accordance with California Title 22 Regulations. A notice of site visit was given and must remain posted for 30 days.
