What the inspector wrote

On April 21, 2026, Licensing Program Analyst (LPA) Monique Ayala conducted an unannounced Case Management - incident inspection at the above facility. LPA met with Education Supervisor, Merwin Caiza-Rodriguez who guided LPA on a tour of the facility. LPA observed 47 children with 8 staff members. The purpose of the inspection is to follow up on an incident report that occurred on 04/09/2026 and was reported to the department on 04/13/2026. During the inspection LPA interviewed Staff #1 (S1) to Staff #4 (S4), Child #1 (C1) to Child #3 (C3) and Parent #1 (P1). P1 stated that C1 informed them that S1 pinched C1 on their back during nap time. Based on interviews, S1 stated that they were not in C1's classroom the day the incident was alleged. Per S1, they were assisting in classroom 2. Per S2-S4, S1 sometimes does come into classroom 1 during nap time to assisting with lunch breaks or provide updates. C1 made no disclosures pertaining to the allegation. P1 stated that they asked S1 with C1 present if the incident had occurred. Per S1, they denied the incident occurred and they immediately reported the concern to their supervisor. P1 stated that they do not think the incident occurred but wanted to inform the facility of the concern. Per P1, they feel that C1 is safe at the facility. S2-S4 and C2-C3 made no corroborating statements. There are no deficiencies being cited at this time. An exit interview was conducted and a copy of this report was provided to Education Supervisor, Merwin Caiza-Rodriguez. A Notice of Site Visit was provided and must be posted for 30 days,
