What the inspector wrote

Licensing Program Analyst (LPA) Lilli Babcock conducted an unannounced Case Management inspection due to an incident that occurred on 12/1/25 and was reported to the Department on 12/2/25. LPA met with Lead Teacher, Toni Morales to whom the reason for the visit was explained. Census was taken. There were two (2) staff present caring for eleven (11) children in Class 01 and three (3) staff present caring for fifteen (15) children in Class 02. The facility was observed to be operating within the license capacity limitations. On December 2, 2025, an unusual incident report was made to the Department regarding an incident that involved a child who sustained an injury that received medical attention. The facility reported this incident to the Department within the required 24 hours. Based on information obtained during interviews conducted with staff and child who was injured, it appears a child tripped over their feet while walking to the trash can to throw away their breakfast. The child fell forward without letting go of the cereal in their hand and hit their nose on the floor which caused the nose to bleed. LPA observed the area where the child was said to have fallen and there appeared to be no tripping hazards. Per interviews with staff and injured child there were no other children next to the child at the time of the incident. Based on interviews with staff, there was adequate supervision at the time of the incident. It appears that although staff were present, they could not prevent the injury. Child was taken to the doctor and Individualized Health Care Plan obtained from facility states the child had a "nasal injury" with limitations including "During outside play, child is only allowed to participate in table time or easel play and cannot perform any physical activity". Per staff, child has returned to day care and restrictions are no longer needed. Page 1 of 2 Based on information obtained during this investigation, no additional follow up is necessary regarding the incident reported. The facility followed proper procedures; staff administered first aid, child\342\200\231s parent was notified, and incident report was sent in properly and timely. No deficiencies are being cited at this time. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Exit interview conducted and report was reviewed with Lead Teacher, Toni Morales. Page 2 of 2
