What the inspector wrote
On 5/22/2025 at 9:00 am, Licensing Program Analyst (LPA), Carolyn Tuba conducted an unannounced inspection visit for the above facility. LPA met with Licensee, Ines Mayorga and spouse, Marcelo Mayorga. LPA was guided on a tour of the inside and the outside of the home and obtained a census of 6 children in care during the visit. Per Licensee there are 8 children currently enrolled. Licensee speaks both Spanish and English.
A current children’s roster is available. Per Licensee, operating hours are from Monday-Friday 6:00 am – 6:00 pm will not exceed 24-hour care at any time and cares for children ages birth – 5 years old.
The facility is a large family child home and LPA provided an Entrance Checklist form. LPA observed all the required documentation on the parent board. Reporting Requirements and Unusual Incident Report (UIR) LIC 624B was reviewed and a UIR for Family Childcare was printed for future use. The Department must be notified within the first 24 hours of the incident or by the next business day and the UIR may be presented in person, mailed, faxed or sent electronically to the following email address:

Licensee states that she is not part of a food program and provides the following meals: Breakfast, morning snack, lunch, and afternoon snack for children in care. LPA advised that if parents do provide food, baby food, and formula that it must be labeled with child’s name and stored appropriately. LPA advised that breast milk should have the date and child’s name written on the bottle.

LPA entered the home through the garage which is used as the entrance for parents to drop their child off. This is a Two-story home which consists of 4 bedrooms, total of 3.5 bathrooms (1 bathroom is downstairs that children use), kitchen, daycare room, living room, front yard and backyard (fenced with a concrete wall and iron gate).
The children use the large daycare room, bathroom attached to daycare room, and backyard.
Per licensee, areas off limits to children and parents include: second floor which includes 4 bedrooms, 2.5 bathrooms, kitchen, and front living room.
All areas identified on the facility sketch were inspected.
The licensee states that 2 adults and no minor children currently live in the home. Licensee states that she currently has one assistant (spouse). All individuals present in the home have obtained a criminal record clearance or exemption prior to working, residing or volunteering in a licensed home. Per licensee, there are no pets, weapons, firearms or bodies of water on the premises. LPA did not observe any at the time of the visit.
The home was inspected for safety, comfort, cleanliness, ventilation, and telephone service. LPA verified the phone number and email address, which was found to be correct. Fire and safety drills must be performed every 6 months and documented for review by the Department. Licensee states she had not conducted an emergency drill or documented since October 2024 and must be completed and documented at least every 6 months. No one smokes at the family childcare home. There are many toys and equipment for children in care to use. There is a changing table for children in care. Per licensee, a side of the daycare room will be used as an area for an ill child until an authorized representative comes to pick up child.
In the absence of the licensee a qualified adult must be present supervising the children; a qualified adult is an individual who has a valid and current Pediatric First Aid and CPR training EMSA approved, Immunization's (TDAP, MMR, TB, Influenza (or a waiver for the influenza may be in writing), mandated reporter training and a valid criminal record clearance associated to the facility license.
Detergents, cleaning compounds, medications, and other items, which could pose a danger, were determined to be inaccessible to children in care during this inspection. Cleaning supplies and kitchen knives were kept inaccessible to children in care. The kitchen area and front area of the home is separated by a child proof gate. LPA did not observe any Medication/Vitamins but advised that they should be made accessible to children. According to licensee there are no children on medication at this time.

LPA observed the required 2:A-10:B-C fire extinguisher, and observed a receipt attached that was purchased 9/21/2024 and per Licensee she purchases a new one each year. State Fire Marshall standards, state that fire extinguisher shall be purchased or serviced annually.
The combo smoke and carbon monoxide detector were tested and operating properly during the visit.
LPA consulted with the Licensee about the 15-minute sleep log that requires to be conducted for children under 24 months and provided the Safe Sleep regulations. LPA observed that the licensee has a copy of the Individual Infant Sleeping Plan (LIC 9227) for children under 12 months of age. LPA advised licensee on PIN 20-24 CCP Safe Sleep. The licensee has completed training on preventive health & safety practices.
Pediatric First Aid and CPR, which includes the EMSA, expired on 5/21/2025 (Tech Advisory issued) and licensee is in the process of signing up for a new class. There are first aid supplies available. Licensee and the assistant’s Mandated Reporting Training Certificate expired on 5/20/2025 (Tech Advisory issued). LPA advised that all persons working with children require the AB 1207 Mandated Reporting training which needs to be completed every 2 years. The following link

Children files must be maintained and updated as needed and must be available for review by the Department. If children and staff leave the day care all records must be maintained for 3 years after leaving. LPA observed that children’s files were complete and advised that the following documents of the LIC 700, LIC 627 and LIC 995A are required, as well as the immunizations. LIC 282 is also needed if liability insurance is not available.

Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed page 3 of 4
Family Childcare Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. LPA reviewed clearance list.
LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at

as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at

and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain childcare by connecting them to childcare providers and Resource and Referral Agencies (R&Rs) throughout California.
During the exit interview, the Licensee, confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS.
The deficiency listed on the following page were observed by the LPA and is being cited in accordance with California Code of Regulations Title 22. Please see attached LIC 809-D. The deficiency that is being cited needs to be cleared to protect the children’s health & safety.
A notice of site visit was given and must remain posted for 30 days. An exit interview was conducted, and report was reviewed with the Licensee, Ines Mayorga.
