What the inspector wrote

On May 1, 2025, at 12:10 pm Licensing Program Analysts (LPA) Priscilla Ochoa conducted an unannounced Case Management Inspection – Plan of Correction at the above facility. LPA met with Director, Sondra Wise who guided LPA on a tour of the facility. LPA observed 28 children in care along with the 5 staff members. The purpose of this inspection is to ensure that the facility is in compliance with Title 22 Regulations and the deficiencies cited on 4/24/2025 were corrected. Licensing staff observed and reviewed the following: · Staff 2 obtained Statement Acknowledging Requirement to Report Child Abuse (LIC 9108) and Employee Rights (LIC 9052) on file · Child 2 – Child 4 obtained completed physician’s report on file · Child 1, Child 5 and Child 6 obtained Parent’s rights (LIC 995) on file · Child 2, Child 3 and Child 5 Acknowledgment of Receipt of Licensing Reports (LIC 9224) on file · Child 4 obtained immunization record on file Letters of Deficiencies Citations Cleared were provided for deficiencies corrected. The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00. An exit interview was conducted, and a copy of this report was provided to Director, Sondra Wise. Page 1 of 1
