What the inspector wrote

On 04/30/2026 at 8:30 am, Licensing Program Analyst (LPA) Shushanik Safaryan conducted an unannounced Case Management -Deficiencies visit to the above facility. The Department received information regarding two separate incidents that occurred at different time frames. According to former staff member 6 (FS6)during naptime preparations, staff heard a noise and observed a bookshelf tipping over with toys stored on the shelves. FS6 reported that toys fell onto a child , startling the child and causing them to cry. Photographs were submitted to LPA for review. During the interviews, The Director confirmed that an incident occurred but stated that only a few toys had fallen. The Staff member 4 (S4)reported observing toys on the floor. The staff member 2(S2) indicated that they tilted the shelf to retrieve a bottle and made a noise but did not recall knocking shelf or toys. The Director reported shelf was emptied and removed from the area. Based on the information obtained , the shelving unit was identified as a three level shelve unit placed along the divider on the carpeted area between classrooms. The unit was not secured and contained toy bins of toys. Staff reported that carpeted areas were utilized for naptime. The LPA was unable to interview a child involved, as a child was transferred to different school .However, LPA talked to the parent. The parent reported that child stated items fell on them, causing them to feel startled and cry. The parent also reported they were not notified of the incident until they brought it to the Director`s attention. Page 1 of 2 Additionally, former staff reported a separate incident involving a glass table. According to the former staff, a child was walking to the restroom when the child struck their head on the glass table. Staff reported that the child sustained deep cut. Photographs of the glass table and injury were submitted to LPA for review. The Director stated that the glass table was removed following the incident. LPA spoke with the parent of child involved in the incident , who confirmed that the child had a deep cut and received medical attention. Based on the information provided, glass table was not age appropriate and accessible to the children in care when children were going to the bathroom. As a corrective measure, Director removed glass table from the facility and placed shelf towards the walls and anchored them to the wall. Based on the LPA’s observations and records review, the following deficiencies listed on the attached LIC 809D (deficiency page) are being cited in accordance with California Code of Regulations Title 22. Deficiencies that are being cited need to be cleared to protect the children’s health & safety. LPA discussed all the deficiencies cited during today’s inspection with the licensee. Per licensee, she understands all the deficiencies and plans of correction . A notice of site visit was given and must remain posted for 30 days.
