What the inspector wrote

Licensing Program Analysts (LPA) Anomeh Eivazian and Veronica Marinez-Garza conducted an unannounced case management- Licensee initiated inspection on 06/21/2022 at 1:00 PM due to licensee is requesting to take out toddler option from infant license and only be licensed for 20 infants. LPAs met with Marita Monfared, director. Director guided LPAs on a tour of the facility. During this inspection at 2:00 p.m. licensee, Sondra Wise arrived at the facility. At this time they have a capacity of 20 infants (12 infants and 8 toddlers) and which to be to 20 infants only. This is an infant program which consists of 2 classrooms. There is also a preschool program on the premises with facility number 198015235. The Infant program operates Monday – Friday from 6:30 AM – 6:30 PM. Areas measured were two infant classrooms and also measured was the entire outside infant play area. Parents enter the facility through the main entrance door which leads to the classrooms. Based on LPAs measurements taken on this date and observations of the facility, the total indoor capacity of two infant classrooms can accommodate 20 infants. The measurements obtained for the entire outdoor space can accommodate a total of 18 infants. There was a approved Fire Clearance from the Glendale Fire Department on file that was granted on 03/10/2010 for capacity of 20. The following corrections will need to be made by 07/22/2022 prior to license update approval. 1) Play yard needs more space or a request for a waiver for a staggered play schedule. 2) Submit a waiver for shared bathroom among infants and preschoolers. REPORT CONTINUES ON NEXT PAGE 1 of 2 At this time, the licensee is in compliance with California Title 22 Regulations. Therefore, there are no citations being issued today. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
