What the inspector wrote

On May 16, 2025, at 1:30pm Licensing Program Analysts (LPAs) A. Carter and J. Duran conducted an Unannounced Annual / Random inspection and met with licensee Alma Madrigal . LPAs disclosed the purpose of the inspection and was granted entry into the facility. Licensee was provided a copy of the Facility Entrance Checklist. All adults associated to the home were found to have criminal record clearance. At the initial start of today\342\200\231s inspection there were (4) day-care children and one assistant present. Licensee states that there are currently (9) children enrolled. Licensee provides care to children ages ranging from 18 months to 12 years of age. Licensee reports the facility\342\200\231s hours of operation are Monday through Friday from 7:30am to 5:00pm. \302\267 At 2:23pm assistant arrived with 3 school age children This is a single story home which consists of (2) bedrooms, (2) bathrooms, living room, kitchen, family room (main care area), and detached garage. Areas accessible to children includes: the family room (main care area- classroom), hallway and outside bathrooms, and kitchen in passing. Areas off limits to children and parents are: (2) bedrooms, and detached garage. The licensee does understand that licensing staff may have access to off-limit areas during inspection visit if necessary. All areas identified on the facility sketch as accessible to children were inspected to ensure that they are clean and orderly with ventilation and heating for the safety and comfort of the children. LPAs observed required posted documentation which included: Facility License, Publication (PUB) 394- Notification of Parent Rights and Licensing Form (LIC) 9148- Earthquake Preparedness form. ..... Report Continues Page 1 of 4 Page 2 of 4 LPAs observed completed facility records including: LIC 9040- Facility Roster, LIC 610- Facility Disaster Plan and Disaster drill log, last drill conducted on 05/16/25. Licensee was that disaster drills shall be conducted every 6 months per regulation. Dual smoke/carbon monoxide detector observed but tested due to children napping at the time of inspection. LPAs observed the AB10BC fire extinguisher with last service date of June 25, 2024. The facility has window a/c units and maintains telephone service via landline and cell phone. LPAs observed age appropriate toys and learning materials available for children in care, free of loose and sharp parts. LPAs observed a wall heater with service tag indicating gas turned off. No fireplaces observed. The children use the bathroom in the hallway. LPAs did not observe hazards in the bathroom. Children nap in the main. LPAs observed napping mats available for children in care. Detergents, cleaning compounds, and medications are stored in an off-limits area of the home inaccessible to children in care. The Licensee states that there are no poisons in the home. The Licensee does understand that poison must be locked. The Licensee was advised that inaccessibility of hazards must be constantly reassessed depending on the children in care. The isolation area for sick children waiting to be picked is the living room couch, supervised away from the other children in care. Per licensee, food is provided for children in care. LPAs reminded licensee that any food brought from the children\342\200\231s homes shall be labeled with child\342\200\231s name and properly stored or refrigerated. Per licensee, the children will have access to the backyard for outside play. LPA observed age appropriate toys for children in care such as a play structure with swings and a slide, toddler size cars, a playhouse, and bikes. The play area is partially covered, providing shading for children in care. Per Licensee, children are supervised while playing outside. LPA observed a locked shed and adequate perimeter fencing through-out the property. Play area is free of loosse and sharp articles or objects and all trees, shrubs, and plants are maintained. LPAs did not observe any pools or bodies of water on the premises. The washer and dryer is located in the backyard as well as an outdoor refrigerator. There are two pet dogs located on the side of the home in the dog run, inaccessible to children in care. Infant Care: Currently there are no infants enrolled at the facility. The following was discussed with the Licensee, Infant mattresses shall be firm with tightly fitted sheets. There shall be no loose object, bumpers, objects hanging, or objects attached to the cribs. Page 3 of 4 Napping equipment shall not block entrances or exits. Licensee was advised Infants shall not be swaddled in care. Car seats shall only be used for transportation purposes and shall not be used for sleeping. LPAs discussed the new Safe sleep regulation, including LIC 9227 Infant Sleep Plan for infants under 12 months, 15-minute sleep check documentation for infants 0 -24 months. Overnight Care : There is no overnight care provided at the moment. LPAs discussed the following: Licensee must remain awake while children are awake. If children sleep in separate area from licensee, the door must remain open. If licensee cannot hear children when they wake up, video or audio devices can be used. Children\342\200\231s records were reviewed for (LIC) 282- Affidavit Regarding Liability Insurance, Immunization's Records, LIC 700- Identification and Emergency Information, LIC 627- Consent for Medical Treatment, LIC 995A Notification of Parents\342\200\231 Rights, and documentation of 15-minute Infant Sleep Check (0-24 months). All required licensing forms are in file. Staff records were reviewed: Licensee's Pediatric CPR / First Aid valid until: 05/15/27 and the Assistant's Pediatric CPR / First Aid valid until: 07/01/26. Staff had proof of required immunization's and TB clearance. Licensee's Mandated Reporter training is valid until: 08/06/26 and Assistant's is valid until: 12/06/25. All adults had LIC 508 criminal record statement on file. Assistant had LIC 9052 on file. During the inspection, children present was observed to be treated with dignity and respect, observed to be receiving safe, healthful, and comfortable accommodations, furnishings, and equipment, and free from corporal and/or unusual punishment. o Licensee stated they do not have any children that needs medication administer, or any children with allergies in care at this time. o Licensee stated that there are no smokers in the home. o Licensee states there are 2 pet dogs in the home observed by LPAs at time of inspection. o Licensee stated there are no weapons, or firearms in the home. o Licensee stated there are no bodies of water around the premises. LPA did not observe any bodies of water at time of inspection. Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02- CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. During the exit interview, the LICENSEE Alma Madrigal, confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS. There are no deficiencies cited today in accordance with California Title 22 Regulations. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the licensee Alma Madrigal.
