ANNUAL / RANDOM INSPECTION CONDUCTED On May 20, 2026, at 11:57 AM, Licensing Program Analyst (LPA) A. Carter conducted an Unannounced Annual / Random Inspection and met with the Licensee Alma Madrigal . LPA disclosed the purpose of the inspection and was granted entry into the facility. LPA was taken on a guided tour of the facility and provided licensee with a copy of the Facility Entrance Checklist. Adults in the home were discussed, and all have criminal record clearance. Also present was Licensee’s Assistant. There were 4 children present during today’s inspection and currently 10 children enrolled. LPA observe children eating lunch. The children’s roster was reviewed by LPA and is current. Hours of operation are Monday through Friday from 7:30 AM to 5 PM. · At 12:55 PM, assistant provider returned with 3 school-age children This is a single story home which consists of 2 bedrooms, 2 bathrooms, and detached garage. Areas accessible to children includes: the family room (main care area- classroom), living room, hallway and outside bathrooms, and kitchen in passing. Areas off limits to children and parents are: 2 bedrooms, and detached garage. The licensee acknowledged that children may never enter these off-limit areas and understands that licensing staff may have access to off-limit areas during inspection visits if necessary. All areas identified on the facility sketch as accessible to children were inspected to ensure that they are clean and orderly with ventilation and heating for the safety of the children. There is a working telephone maintained in the home via cellphone and landline. The facility has central air & heating. LPA observed hallway wall heater with service tag indicating gas turned off. Facility License, Emergency Disaster Plan, and Parents’ Rights were posted and observed at time of inspection. Disaster drill log was available during today’s inspection with last disaster drill conducted on 01/26/26. LPA observed the required 2A10BC fire extinguisher with service tag dated: 07/24/25. Dual smoke and carbon monoxide detectors observed and tested at time of inspection. First Aid kit is mounted on the wall in the main care area observed by LPA. LPA observed the living room to have a sectional couch, mounted TV, child size table with rounded edges and chairs, a highchair, a play yard, and cubbies for the children’s belongings. LPA observed the classroom (main care area) has age-appropriate toys, free of loose and sharp parts. The classroom has a changing table against the wall, library area, dramatic play area, cubbies for the children’s belongings and child size tables and chairs. Children nap in the classroom and they are provided with appropriate napping equipment. Children use the bathroom located in the hallway. LPA observed bathroom to have operable toilet/sink and adequate toileting supplies for children in care. No hazards observed. Food is provided for children in care. LPA reminded licensee that any food brought from the children’s homes shall be labeled with child’s name and properly stored or refrigerated. Knives are kept inaccessible to children in care on an upper shelf near the kitchen window and cleaning products are kept in a locked upper cabinet. Personal medications are stored in an upper cabinet secured by safety latch. Detergents and other cleaning compounds are stored in off-limit areas of the home. The Licensee states that there are no poisons in the home and understands that poisons must be locked. Per licensee, the children have access to the backyard for outdoor play. LPA observed age appropriate toys for children in care such as a play structure with swings and a slide, toddler size cars, a playhouse, and bikes. The play area is partially covered by a tent, providing shading for children in care. Per Licensee, children are supervised while playing outside. The washer and dryer is located in the backyard as well as an outdoor refrigerator. LPA observed outdoor bathroom to have operable toilet/sink and adequate toileting supplies for children in care. No hazards observed. There is a pet dog located on the side of the home in the dog run, inaccessible to children in care. LPA observed adequate perimeter fencing throughout the property. Play area is free of loose and sharp articles or objects and all trees, shrubs, and plants are maintained. LPA did not observe any pools or bodies of water on the premises. .........................................................Report Continues 3 of 5 Pages....................................................... Infant Care: Currently, there is one infant over 12 months enrolled. LPA reviewed sleeping log. The Licensee was advised; infants shall not be swaddled in care. Car seats shall only be used for transportation purposes and shall not be used for sleeping. LPA discussed the Safe sleep regulation, including LIC 9227 Infant Sleep Plan for infants under 12 months, 15-minute sleep check documentation for infants 0 -24 months. —Children’s records were reviewed for (LIC) 282- Affidavit Regarding Liability Insurance, Immunization's Records, LIC 700- Identification and Emergency Information, LIC 627- Consent for Medical Treatment, LIC 995A Notification of Parents’ Rights, and documentation of 15-minute Infant Sleep Check (0-24 months). All required licensing forms are in file. Staff records were reviewed: Licensee and Assistant’s Pediatric CPR / First Aid valid until: 05/15/27 . Staff had proof of required immunization's and TB clearance. Mandated Reporter Training Certificate for the Licensee is valid until: 08/06/26 . Assistant did not have Mandated Reporter Training on file, technical violation cited. All adults had LIC 508 and LIC 9052 on file. The following was discussed: · Rooms that are off-limits need to be made inaccessible during operating hours. NO smoking, NO infant walkers, NO Johnny jumpers, NO saucer chairs, NO incline sleepers, or any other item that falls into that category are permitted in the facility. · Effective January 1, 2010, Licensees of family childcare homes are required to ensure that at least one staff member with current training in Pediatric First Aid & CPR is on site at all times when children are present. · Reporting Requirements: Changes should be reported to the Department as soon as they occur, such as construction, remodeling, telephone number changes and/or if you move from your home. Any unusual incidents or injuries must be reported to the Department within 24 hours via telephone and within seven (7) days in writing. The licensee confirms: The isolation area for sick children waiting to be picked up is the living room. The facility does not currently administer medication. There is 1 pet dog in the home that do not interact with day care children. There are no smokers in the home. There are no weapons or firearms in the home. There are no pools, spas or other bodies of water on the premises. During the inspection, children present were observed to be treated with dignity and respect, observed to be receiving safe, healthful, and comfortable accommodations, furnishings, and equipment, and free from corporal and/or unusual punishment. .........................................................Report Continues 4 of 5 Pages....................................................... Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for and removing any recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22- 02-CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain childcare by connecting them to childcare providers and Resource and Referral Agencies (R&Rs) throughout California. During the exit interview, the Licensee Alma Madrigal, confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS.
