What the inspector wrote

At 1:00 PM Licensing Program Analyst (LPA) Roxana Lopez conducted an unannounced required inspection. A risk assessment was conducted prior to entering the facility appropriate PPE was used. LPA met with Lead Teacher, Christina Molina, who guided the LPA on a tour of the facility. This is a preschool age head start program licensed for 40 children which operates Monday \342\200\223 Friday 8:15 am to 3:45 pm. Room # H1 operates full day and Room # H2 operates two half sessions Per Lead Teacher there are 54 children enrolled total. Education Specialist, Maricela Arellano arrived at 2:00 pm and took over inspection. All areas identified on the Facility Sketch were inspected. Facility is located in 2 portable classrooms on the Bella Vista Elementary School Campus. The following staff was present during this inspection: Room # H1: Staff #1 and Staff #2 with 15 children and Room # 2: Staff # 4 and Staff # 5 with 14 children. Teacher-child ratios were observed to be in accordance with Title 22 Regulations. Staff names were recorded. All children were observed to be under supervision, including visual supervision, of a teacher at all times. The licensed facility is within the conditions, limitations, and capacity specified on the license. LPA observed the facility to be clean, sanitary and in good repair. Furniture and equipment was inspected for good repair, free of sharp, loose, or pointed parts. Classroom was inspected to ensure that the floors have a surface that is safe and clean. All toilets and washing facilities are in safe and sanitary operating conditions. All materials and surfaces accessible to children are toxic free. At this time, the teachers office is used as an isolation area. ------------------------------------------------- pg. 1 of 4 ---------------------------------------------------------------- A mat is available if needed and staff restroom is available to use if needed. Parents are contacted immediately when children are determined to be ill and staff are ensuring that children with obvious symptoms of illness are not being accepted. The facility provides breakfast, lunch and snack for full day, breakfast and lunch for am session and lunch and supper for pm session. Menus were reviewed to ensure that they are being posted at least one week in advance and visible to an authorized representative. Per lead teacher meals are picked up at the elementary cafeteria. LPA inspected the kitchen- All food preparation, and storage areas are clean, free of litter, rubbish, and rodents/vermin. All food is protected from contamination, and LPA inspected that any contaminated food is discarded immediately. There is drinking water available in the classroom via water dispenser. Water jug and dixie cups are taken outside for children to drink. All storage containers for solid waste, including moveable bins, have tight fitting covers on and are in good repair. Disinfectants, cleaning solutions, poisons and other items that are dangerous to children are stored in an area inaccessible to children. LPA observed that there is smoke and carbon detectors in the facility and were found to be operable. The last Disaster and Fire Drill was conducted on 5/2/2023 LPA observed sleeping arrangements in form of cots for the full day program- sheets are sent home every Friday to be washed. Outdoor playground equipment is in safe condition, free of sharp, loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. All areas around or under high climbing equipment, swings, slides, and similar equipment are cushioned with material that absorbs a fall. There is adequate shade in the play yard. ----------------------------------- ---------------- pg 2 of 4 ------------------------------------------------------------ Parents sign in an out through the child\342\200\231s plus attendance app. LPA reviewed the app to ensure that the person who signs the child in and out uses their full legal signature and records the time of the day There is at least one person trained in CPR and Pediatric First Aid present during this inspection. All staff have received an active criminal record clearance as a condition of their employment with the Montebello Unified School District. In review of children\342\200\231s records, files contain information including, but not limited to the following: Name, address and telephone number of the child's authorized representative and of relatives or others who can assume responsibility for the child if the authorized representative cannot be reached when necessary. Children's roster was reviewed and is current. LPA reviewed files for all staff present. LPA observed that staff #1 \342\200\223 4 did not have proof of LIC 503 Health Screening. AB1207 Mandated Child Abuse Reporting \342\200\223 On or before March 30, 2018 any person who works in a child care facility shall complete the training and renew the training every 2 years. Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.
