What the inspector wrote

On 06/23/2022 at 9:00 am, Licensing Program Analyst (LPA) Roxana Lopez conducted an unannounced required inspection. A risk assessment was conducted prior to entering the facility. LPA met with Site Supervisor, Berenice Marin, who guided the LPA on a tour of the facility. This is a preschool age program licensed for 42 children which operates Monday \342\200\223 Friday from 7:00 AM \342\200\223 5:30 PM. Per the Site Supervisor there are 14 children enrolled. Upon arrival LPA was informed that facility was having a drive up graduation and children would be coming with their families throughout the day. Director Rachel Cordova arrived at 11:30 am. All areas identified on the Facility Sketch were inspected. The following staff was present during this inspection: Room # 1 and Room # 2 3 teachers no children present. Staff names were recorded. The licensed facility is within the conditions, limitations, and capacity specified on the license. LPA observed the facility to be clean, sanitary and in good repair. Furniture and equipment was inspected for good repair, free of sharp, loose, or pointed parts. All indoor classrooms were inspected to ensure that the floors have a surface that is safe and clean. All toilets and washing facilities are in safe and sanitary operating conditions. All materials and surfaces accessible to children are toxic free. LPA observed sleeping arrangements in form of cots. At this time, the office is used as an isolation area. There is a mat available if needed. Parents are contacted immediately when children are determined to be ill and staff are ensuring that children with obvious symptoms of illness are not being accepted. ------------------------------------------------ Pg. 1 of 4 --------------------------------------------------------------- Snack menus were reviewed to ensure that they are being posted at least one week in advance and visible to an authorized representative. The facility provides AM snack, Breakfast, PM snack and lunch. All kitchen, food preparation, and storage areas are clean, free of litter, rubbish, and rodents/vermin. All food is protected from contamination, and LPA inspected that any contaminated food is discarded immediately. There is drinking water available in all indoor classrooms and drinking containers / jugs are taken outdoors. All storage containers for solid waste, including moveable bins, have tight fitting covers on and are in good repair. Disinfectants, cleaning solutions, poisons and other items that are dangerous to children are stored in an area inaccessible to children. Site Supervisor states that there are no poisons stored at the facility. At 9:25 am LPA\342\200\231s observed that there is smoke and carbon detectors in the facility and found them to be operable. The last drill documented was 6/17/2022. Outdoor playground equipment is in safe condition, free of sharp, loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. All areas around or under high climbing equipment, swings, slides, and similar equipment are cushioned with material that absorbs a fall. There is adequate shade in the play yard. Sign in and out sheets were reviewed to ensure that the person who signs the child in and out uses their full legal signature and records the time of the day. Staff Records were reviewed. Children\342\200\231s Records were reviewed to ensure that Identification and Emergency form and a medical assessment are on file. Criminal Records Clearance for adults and verification of CPR/First Aid and health preventative practices documentation was reviewed. LPA issued a copy of the LIC 857 Children\342\200\231s Records Review and the LIC 859 Staff Records Review to the licensee during this inspection. ----------------------------------------------- Pg 2. of 4 -------------------------------------------------------------- SB792 (Immunization Requirements for Staff and Employees) was discussed with the Manager and Administrator. The Site supervisor and her staff do currently have immunization documentation. AB1207 Mandated Child Abuse Reporting \342\200\223 On or before March 30, 2018 any person who works in a child care facility shall complete the training and renew the training every 2 years. LPA reviewed storage of medication and equipment/supplies, and reviewed children\342\200\231s, personnel, and administrative records. At 10:00 am LPA observed that Medication for Child # 1 does not have the original Dr. label and no paperwork is on file. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated. Deficiencies that are being cited need to be cleared to protect the children\342\200\231s health and safety. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
