What the inspector wrote

On Monday, March 23, 2026, a Case Management - Deficiencies visit was conducted during a Complaint investigation to cite for deficiencies found during the investigation. Upon arrival, Licensing Program Analyst (LPA) Staicy Perry was greeted by Wai Yu, Licensee and Sik Yung. Co-Licensee who guided LPA Perry on a tour of the home. Census was taken and LPA Perry observed a total of 7 children in care. Per Wai Yu, Licensee and Sik Yung. Co-Licensee there are currently 10 children enrolled at the facility. Per Licensee currently has 8 children enrolled. During observations, interviews and record review, it was determined that upon arrival, LPA Perry observed only two (2) children in the main day-care area/ Family room, as one child was just being dropped off by parent, but licensee stated that they had seven (7) children in care. LPA Perry was guided to observe the rooms. LPA observed C#1, C#2, C#3 in bedroom located near the day-care area/family room and C#4 and C#5, located in bedroom near the living room. LPA observed and heard children screaming and crying sleeping in cribs in the bedroom near the living room. Children were observed to be in separate bedrooms with the doors closed, lights off, no ventilation and some children were observed to be awake. LPA Perry also observed a child safety gate placed in front of the bedroom door. LPA observed children to be in sleep sacs and blankets and toys hanging in and around the children in the cribs. LPA heard children screaming and crying while inside the rooms. LPA Perry did inform licensee to bring the children out of the bedrooms and into the main day-care room. LPA requested infant sleep logs; however, licensee was unable to provide documentation showing that sleep checks were being conducted. LPA informed both licensees that cribs or play yards shall be free from all loose articles and objects. Licensees understood. LPA also informed licensee that the provider shall supervise infants while they are sleeping and if the infant is sleeping in a separate room from where the provider is stationed, the door to the room the infant is sleeping in shall remain open at all times. Licensee understood an removed the children from the bedrooms. LPA Perry informed licensee that this poses an immediate risk to the health and safety of the children in care. LPA Perry reviewed the facility file and observed the same concern cited on 1/20/2026. The deficiency listed on the following page was observed by the LPA Perry and is being cited in accordance with California Code of Regulations Title 22. Two (2) type A deficiency is being issued today. Please see attached LIC 809-D for citation. Deficiencies that are being cited need to be cleared to protect the children’s health & safety. A copy of this report shall be provided to the parent/guardian of children currently enrolled by the next business day or immediately upon their return. A copy of this report shall also be provided to the parent/guardian of any newly enrolled child for the next 12 months. A signed Acknowledgement of Receipt (LIC9224) shall be in each child’s file, acknowledging receipt. The following is being cited in accordance to Title 22 of the California Code of Regulations and Health and Safety Code of the State of California. One (1) Type B being cited today. Civil penalty has been assessed. Exit interview conducted with Wai Yu, Licensee and Sik Yung. Co-Licensee. Copy of this report provided Wai Yu, Licensee and Sik Yung. Co-Licensee. Appeal rights were provided and explained.

    The provider shall supervise infants while they are sleeping and adhere to the following requirements: The provider shall physically check on the infant every 15 minutes.This requirement is not met as evidenced by: Based on observation and record review, the licensee did not comply with the section cited above in LPA did not observe C#1, C#3, C#4 and C#5 did not have a current 15-minute sleep log; last documented on 3/11/2026, which poses a potential health, safety or personal rights risk to persons in care.

  • INFANT SAFE SLEEP

    The provider shall supervise infants while they are sleeping and adhere to the following requirements: If the infant is sleeping in a separate room from where the provider is stationed, the door to the room the infant is sleeping in shall remain open at all times. This requirement is not met as evidenced by:Based on observation the licensee did not comply with the section cited above in LPA Perry obserevd the bedroom doors closed while infants / children were napping which poses/posed an immidiate risk to the health, safety or personal rights risk to persons in care.
