What the inspector wrote

An informal office meeting was conducted on 08/24/23 at 11:30 AM at the Monterey Park Regional Office. Present during the meeting were Licensing Program Manager (LPM) Christina Gabelman, Licensing Program Analyst (LPA) Anomeh Eivazian and Licensee Sharon Lee. This meeting was conducted in English. The purpose of this Informal Meeting is to discuss the history of the facility. The summary of history includes 8 Type A's deficiencies from 2023-2017. Deficiencies include: On 04/05/2023– Type A—101216.3 (a)—Teacher Child Ratio. On 04/03/23 during staff lunch break, 15 children were supervised by only one staff. On 03/15/2023—Type A—101216.3(a)—Teacher Child Ratio. On 02/17/23 between 12:38 PM to 1:05 PM, one teacher was supervising 23 children alone, at 12:48 PM one teacher assistant joined for assistant. On 03/15/2023—Type A—101223(a)(2)—Personal Right-- On 02/17/23 between 12:38 PM to 1:00 PM on sand area, one child pulled down child’s pants and lifted shirt and teachers did not observe it to prevent the act. On 03/15/23—Type A—101229(a)(1 )-- Responsibility for Providing Care and Supervision-- On 02/17/23 between 12:38 PM to 1:00 PM on sand area, one child pulled down child’s pants and lifted shirt and teachers did not observe it to prevent the act. On 07/22/2022—Type A—101538.3(b)-- Indoor Activity Space for School-Age Children-- On 06/24/22 school age children commingled with preschoolers in RM A between 8:00 a.m. to 8:30 a.m.. On 05/20/2021—Type A--101216.3(a)—Teacher Child Ratio--The facility had 13-14 children in care the week of 02/16/21 - 02/19/21, which is more than the 12 allowed by the Department of Public Health guidance (Covid) during that time frame. On 08/10/2017—Type A--101216.3(a)—Teacher Child Ratio -- LPA observed RM D children, 17 preschoolers were having lunch in the shaded area with 1 teacher assistant. REPORT CONTINUES ON NEXT PAGE 1 of 2 On 08/10/2017—Type A—101170(e) (1)-- Criminal Record Clearance—Two staff did not have criminal record clearance on Licensing Information System (LIS). The licensee has agreed to the following to ensure compliance: 1. The facility must be in compliance at all times. 2. The facility will be placed on required visits for the next two years. 3. The Licensee was advised that it is the responsibility of the Licensee, to know & understand the requirements of Title 22 Regulations. 4. The Licensee will also be required to view the following videos on the Department website https://ccld.childcarevideos.org/ : · Teacher-to Child Ratios in Child Care Centers · Supervising Children in Child Care Centers · Children’s Personal Right in Child Care · Background Check Requirements for Caregivers The Licensee shall submit a written statement to the Department on what she learned from each video no later than 09/25/2023. Licensee will also show the first 3 videos to staff during her monthly staff meeting and submit copy of sign in sheet to the department. 5. The Licensee and facility director will complete Child Care Centers Step II Orientation and a copy of certificate will be submitted no later than 09/25/2023. LPM Gabelman reviewed with licensee, Title 22 Regulation code-- 101215.1 --Child Care Center Directors Qualifications and Duties. LPM Gabelman informed licensee that a full-time director is required to be present at the facility to observe preschool program at all times. Per Sharon Lee, Rima Dibbie is the preschool director also she is Kindergarten classroom full time teacher. However, whenever is needed Rima Dibbie is assisting us with preschool program. Per Sharon Lee, an email blast will be sent out to all the parents at the end of August 2023 and will formally introduce Rima Dibbie as preschool program director and send copy to the department. Licensee was advised if Teacher Child Ratio Requirement is not being met during the compliance period, Licensee may be called in for Non-Compliance Meeting sooner.
