What the inspector wrote
On 11/06/24 , 8:30 am ,Licensing Program Analyst (LPA) Shushanik Safaryan conducted an unannounced Case Management -Legal visit due to receiving Stipulation and Waiver , and Order for the above facility signed on 09/23/2024. Upon arrival , LPA met with Director Sharon Lee to whom the reason of the inspection explained and who guided analyst on a tour of the facility. This is a preschool program, licensed for 105 children which operates Monday-Friday from 7 AM to 6 PM.
Director stated she has already received the Stipulation and Waiver , and Order with the effective date 09/23/2024. Copy was provided during this visit too.
Per Stipulation and Waiver and Order dated 09/23/2024 director understands facility granted a probationary license for three years and subject to the following limitations and conditions:
-Facility will operate strict in compliance with the regulations and statues governing the operation of a childcare center.
-During the period of probation , the Department may conduct unannounced site visits for the purpose of determining whether their full compliance with regulations and statues.
-Facility shall install and maintain appropriate safety devices on all classroom doors which shall promptly alert facility staff when said door opens ,such as an alarm , door chime, or similar device.
-Facility director or any individuals who have been or will be delegated with decision making authority to act or on behalf of the facility director within six months of the adoption of Stipulation complete 40 hour of training related to allegations . Training topics must include :Care and Supervision, Personal Rights, Active Supervision.
-Facility is required to maintain an accurate , complete, and current client roster which must be made available to the Department upon request .

- Facility shall develop, implement, and maintain sign in and sign out system regarding children attendance . Facility shall keep the records of said sign in and sign out system and provide such records to Licensing staff upon request.
-Facility shall develop , implement, and maintain “head count” system whereby they document the presence of each child in attendance at the start of every hour, during each transition period and whenever staffing shift change. Respondent should also review the “head-count” system for effectiveness in ensuring children safety and supervision whenever changes to staff schedules are implemented. Facility shall keep the records of said head count system and provide such records and/or revisions to Licensing staff upon request .
-Facility
shall report to the Licensing office the following: any unusual incident including, but not limited to, client death or injury which requires medical treatment, any suspected physical or psychological abuse of any client, any physical plant changes, and all unexplained absences. These incidents must be reported by the next working day, and a written report of the incident must be submitted within seven days following the occurrence of the incident.
-Facility need to provide direct and active supervision all the time. Said supervision shall include , among other things , appropriate monitoring, and supervision of facility grounds .
-Facility need to provide annual training to the any individuals who has authority to act as or on behalf of the facility director and staff applicable Title 22 regulations including active supervision and shall keep a record of all individuals who attend such a training and provide such records to Licensing staff within sixty days of completion of said training.
-Respondent agrees , during probationary period, shall not apply for , receive, or hold ,an administrator certificate or any license or certification to operate , any facility licensed by the Department of Social Services.
-Respondent understands and agrees
that the Department, Foster Family Agency, or a county may deny any application for licensure, certification, registration, or approval submitted after the probationary period, in whole or in part based upon the findings in this Stipulation.

-The probationary period is tolled during any period when the facility is not operating. The probationary period shall be extended by the total time during which the facility is not operating.
Upon a finding that a condition or term of probation or any of the other terms of this Stipulation was violated, the probationary license shall be revoked, and Respondent shall be excluded from any resource family home and from facilities licensed by the Department.
If Respondent has successfully complied with the terms of this Stipulation, at the end of three years from the effective date, the conditions imposed upon Respondent and Respondent's license will expire, and Respondent's license shall be granted or restored in full.
There were no
deficiencies cited during today's visit in accordance with the California Code of Regulations Title 22, Division 12, Chapter 1.
A notice of site visit was given and must remain posted for 30 days.
