What the inspector wrote

Licensing Program Analyst (LPA) Alicia Mooberry conducted an unannounced case management inspection to follow up on incident that was reported to the Department regarding Staff #1 (S1) that had a medical incident during outdoor playtime. There were 99 children and 12 staff during this visit. Interviews with staff and director confirmed that on 5/8/25 at 4:15pm S1 had a medical episode that rendered S1 unable to care for children. 6 children were present during medical episode and were removed from playground by Jocelynn Leon Assistant Director. Interviews with staff confirmed that the facility was within ration and supervised during the incident. The facility reported the incident to the Department within the required 24 hours. Records of S1 was reviewed, S1 is no longer working at the facility, last day was 5/19/25. Record reviews show that the Director took measures to ensure S1 provided an updated LIC 503 Health Screening. Based on the information obtained and LPA observation, and record review, LPA determined there will be no deficiencies to be cited at this time, per California Code of Regulations Title 22, Division 12, Chapter 1. The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative.
