What Happened

Case Management investigation found facility staff out of ratio during nap time with multiple children awake (1-2 per room) and only one teacher per room. Director unaware of ratio requirements when children were not fully asleep.

Observations

  • Room #1: 24 children with 1 teacher; 2 children awake (1 sitting on cot, 1 walking)
  • Room #2: 22 children with 1 teacher; 1 child awake (walking)
  • Room #3: 23 children with 1 teacher; 1 child awake (in cot)
  • Director Cindy Sinsun called for coverage which arrived within 5 minutes
  • Director unaware that staff were out of ratio with only one child awake
