What the inspector wrote

On June 25, 2025, at 9:15a.m., Licensing Program Analyst\342\200\231s (LPA\342\200\231s) Peter Bishop and Alicia Mooberry arrived at the above facility for the purpose of an unannounced/annual visit. LPA\342\200\231s announced the purpose of the visit and was granted entry into the facility by Director - Shannon Batista. There are 15 children enrolled, and 11 children present, and 1 staff present at the time of inspection. This is a School Age Program with the hours of operation Monday- Friday from 6:30a.m-6:00p.m. All adults at the facility were discussed and background and fingerprinted cleared. LPA observed all posting requirements for operation on the licensing board LIC 203A-License, LIC 610, PUB 393, PUB 269, LIC 9148, LIC 613A, Menus, Activity Schedule, and completion testing and remediated lead exceedances. Documentation of current Fire & Earthquake drill was observed with last drill conducted on 5/21/2025. Fire extinguisher(s) observed with a service tag date of 06/2025. LPA observed an operable smoke detector, and carbon monoxide detectors to be operational. At 10:00 a.m. LPA\342\200\231s inspected the classroom. There is one classroom that is used for the School Age Component. The classroom was equipped with age-appropriate furniture and learning materials. Classrooms has carpeted area, sink, soft furnishings, cubby storage for shoes, age-appropriate toys. The facility uses children cups bought from home as a water source and there are water fountains available in the classroom and in the outside play area. The bottles are refilled throughout the day as needed. The bottles are also Page 1 of 5 taken outside during outdoor play. The children do nap on nap mats that are stored in a closet in the front of the classroom. The First Aid supplies were inventoried in classroom backpack. All cleaning supplies and/or hazardous materials stored in cabinets and drawers with safety latches. LPA observed classroom, equipment, and materials to be in good condition, free of sharp, loose, or pointed parts. The classroom has a Bathroom with 2 age-appropriate sinks and 2 toilets. The bathroom was in good working condition. There was hole in the wall observed in the classroom. Director stated she would cover the hole and a workorder has been placed. The Playground area has retractable canopy for shade, small climbing structure, small slide, basketball court, artificial soft turf is under the climbing structure. LPA\342\200\231s observed Playground area to be in good condition, free of sharp, loose, or pointed parts. Although there was a chair that could be of harm to children because of the splintering wood. The chair was removed by Licensee immediately before children came outside. All electrical outlets are covered. All trash cans have lids. The facility does provide breakfast, lunch, and snack. LPA observed the kitchen area where food is distributed to be clean and organized. At 10:30am, LPA Bishop observed in the kitchen cabinets and refrigerator to have food items stored in refrigerator and freezer with labels and current dates. However, there were several containers of oatmeal that were expired more than 2 years old it was determined that the oatmeal is good and verified by FDA. LPA\342\200\231s discussed with Licensee the importance of dating items once removed from packaging and making sure the items or safe for consumption and use. At 10:45am, LPA Bishop observed sign in and out sheets for all signatures were present and reflected the number of children observed at the time census was taken. The facility was reminded that smoking is prohibited on the premises. The isolation area is in the Directors office when any child is sick. The Director stated they do not administer medication to at this time. Page 2 of 5 The facility uses a landline. The Director stated there are no firearms at the facility. The Director stated that they do not provide transportation for children. There are no large bodies of water on premises. Staff Files Staff #1 File has all required licensing documents and immunization record in folder. CPR expiration date of 2/2027 Mandated Reporter expiration date of 8/8/2026 Accredited courses in file. Staff #2 File has all required licensing documents and immunization record in folder. Criminal Record Clearance - Child Care Centers- Director - Shannon Batista was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. Lead Testing Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For childcare center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1- CCP). LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP. Incidental Medical Services (IMS) Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02- CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. MyChildCarePlan.org Director - Shannon Batista was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain childcare by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. Page 4 of 5 Based on the LPA\342\200\231s observations, record review, and interviews, there will be 1 deficiency cited today in accordance with California Title 22 Regulations. There will be one Technical violation as well. Based on this information, the following deficiencies on the attached LIC 809D are being cited in accordance with California Code of Regulations Title 22. Deficiencies that are being cited need to be cleared to protect the children\342\200\231s health & safety. A notice of site visit was given and must remain posted for 30 days.
