What the inspector wrote

Licensing Program Analyst (LPA) Lilli Babcock conducted an unannounced Case Management inspection due to an incident that occurred on 3/12/26 and was reported to the Department on 3/12/26. LPA met with Office Administrator, Cynthia Alvarez to whom the reason for the visit was explained. Census was taken. There were 7 staff present caring for 44 children. The facility was observed to be operating within the license capacity limitations. On March 12, 2026, an unusual incident report was made to the Department regarding an incident that involved a meeting with facility personnel, behavioral therapist, and parents of Child #1. The facility reported this incident to the Department within the required 24 hours. No deficiencies are being cited at this time. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Exit interview conducted and report was reviewed with Office Administrator, Cynthia Alvarez.
