What the inspector wrote

Licensing Program Analyst (LPA) Lilli Babcock conducted a Case Management-Deficiencies visit on 7/24/24 to address deficiencies revealed during a complaint investigation. A COVID risk assessment was conducted. LPA met with Assistant Director, Magali Perez, to whom the reason for the visit was explained. LPA observed eighteen (18) children and five (5) staff present at the facility during this inspection. On 6/19/24 and 7/24/24 LPA Babcock visited the infant facility to conduct a complaint inspection for the infant license. Staff interviews and documents received during the complaint investigation revealed the infant facility has fed infants by propping a bottle up with a blanket or other item. During the complaint investigation for the infant program, LPA conducted interviews with Assistant Director and six (6) staff. LPA Babcock also obtained several documents related to the complaint allegation, including but not limited to, a copy of the Facility Roster (LIC 9040), and Personnel Report (LIC 500). During interviews, Staff #1 and Staff #5 stated they have observed a child in care that cannot hold a bottle by themselves, to have a bottle being propped up by something. Based on staff interviews, the following deficiencies listed on the attached LIC 809D (deficiency page) are being cited in accordance with California Code of Regulations Title 22. Deficiencies that are being cited need to be cleared to protect the children’s health and safety. A notice of site visit was given and must remain posted for 30 days.Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Exit interview conducted and report was reviewed with Assistant Director, Magali Perez.
