What the inspector wrote

Licensing Program Analyst (LPA) Lilli Babcock conducted a Case Management-Deficiencies visit on 4/23/26 to address deficiencies revealed during a complaint investigation. LPA met with Director, Cynthia Alvarez, to whom the reason for the visit was explained. LPA observed twenty three (23) children and eight (8) staff present at the facility during this inspection. LPA Babcock visited the infant facility on 1/30/26, 2/5/26, 4/22/26, and 4/23/26 to conduct a complaint inspection for the infant license. Staff interviews conducted during the complaint investigation revealed the facility has used inappropriate language with the children during diaper changes/toileting. During the complaint investigation LPA conducted interviews with nine staff. During interviews conducted staff were asked if they have heard of a staff shaming a child while changing their diaper or otherwise, saying "stinky" or any other phrases or gestures. Staff #3 stated, "Ya I have heard that in a kinda shaming way". Staff #6 stated, "Yes. I have heard "you stink, caca" from a staff and I told her not to do that". Based on interviews conducted, the following deficiency listed on the attached LIC 809D (deficiency page) is being cited in accordance with California Code of Regulations Title 22. Deficiency that is being cited need to be cleared to protect the children’s health and safety. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Exit interview conducted and report was reviewed with Director, Cynthia Alvarez. Page 1 of 1
