What the inspector wrote

On 11/16/2022 at 9:10 AM, Licensing Program Analyst (LPA) Carolyn Tuba conducted an unannounced required annual inspection. LPA met with Assistant Director, Maddie York. A COVID-19 risk assessment was conducted prior to entering the facility. At 10:00 AM Director, Adetayo Abon arrived to the facility. Director then guided LPA on a tour of the facility indoors and outdoors at around 10:30 AM. This is a preschool age program (2-5 years old) licensed for 37 children, which operates Monday through Friday from 6:30AM to 7:00 PM. Per the Director, there are 62 children enrolled who come at various schedules, but at no time does the facility have more than the licensed capacity of 37 children. The following classrooms were inspected: ABC classroom, which is one big room that accommodates the children. There is a quiet area located off of the main room. The isolation room used is the Director’s office, which has a staff bathroom right off the office side door. There is a employee lounge located off of the main lobby near the Director's office. Teacher-child ratios were observed to be in accordance with Title 22 Regulations. Staff names were recorded. All children were observed to be under supervision, including visual supervision. Licensed facility is within the conditions, limitations, and capacity specified on the license. The classroom furniture and equipment were inspected for good repair, free of sharp, loose, or pointed parts. All indoor classrooms were inspected to ensure that the floors have a surface that is safe and clean. There are 3 working toilets and 3 sinks, one of which is currently not being used. All toilets and washing facilities are in safe and sanitary operating conditions. All materials and surfaces accessible to children are toxic free. Page 1 of 4 The facility provides morning and afternoon snack and lunch is brought from home. The children if hungry at the end of the evening since they close at 7 pm have the option to have a small snack or if they have any leftovers from lunch time. The food is prepared and kept in the kitchen area with expiration dates. A 30 day snack menu is posted right outside of the main classroom for parents to observe and a menu is posted in the kitchen area. Licensing staff observed that there are functioning smoke and carbon detectors in the facility. Licensing staff observed a fully charged and serviced fire extinguisher last serviced on 9/8/2022. Emergency disaster drill was last conducted on 11/2022. There are First aid kits, which are kept in the kitchen area, office area, and outside; the supplies were inventoried and found to be complete. Outdoor playground equipment is in safe condition, free of sharp, loose, or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards, however LPA advised Director that play structures and walking pathways be swept on a regular basis. All areas around or under climbing equipment, slides, and similar equipment are cushioned with a soft foam material that absorbs falls, however there was a small area near the swing set that requires sand to be leveled and filled. LPA advised that staff skim through the sand areas daily and inspect for animal feces and or any unsafe objects. There are several trees that provide adequate shade in the play yard. There are tricycles stored in the shed near the play yard. Area is gated with high iron gates and there is a side entrance that parents may use, that require a code to enter. There is a blacktop area and a tri-level basketball structure for children to play. Sign-in and out is done electronically with a QR code that is scanned by the authorized individual using their cell phone called Smartcare. Parents and authorized individuals have an app and are able to sign their child in and out each day. Facility is able to print a report and was reviewed. A paper sign-in and out sheet is used if a parent forgets to bring in their phone. Page 2 of 4 Licensing staff reviewed staff records and they were complete. Clearance for adults and verification of CPR/First Aid and health preventative practices documentation was reviewed. Children’s records were reviewed to ensure that Identification and Emergency form and a medical assessment are on file. The Director and staff currently have immunization documentation. AB1207 Mandated Child Abuse Reporting – On or before March 30, 2018 any person who works in a childcare facility shall complete the training and renew the training every 2 years. Per Assistant Director Medication is kept in a container in the kitchen, kept in a high-up cupboard and an IMS plan is available in the child’s file. Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. Director was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.
