What the inspector wrote

On 06/23/2025 an unannounced Case Management – Deficiencies inspection was conducted by Licensing Program Analyst (LPA) Carolyn Tuba who was there to conduct a separate visit. During the course of a previous investigation on 4/30/2025 a deficiency was disclosed during interviews with staff. LPA met with Assistant Director, Alicia Rodriguez and obtained a census of 30 children present with 5 staff and a Behavioral Therapist. LPA confirmed that staff was fingerprint cleared. Director, Adetayo Akiwowo arrived at approximately 2:45 pm and was included in the review of the report. During the visit on 4/30/2025 at approximately 10:30 am. LPAs Tuba and Diana Ortiz conducted an interview with Assistant Director #2 who disclosed of an incident between Child #1 (C1) and #2 (C2) that occurred on the playground on 4/8/2025 in the late afternoon and should have been reported to the Department. Assistant Director #3 and Staff #4 (S4) confirmed the incident occurred. LPA consulted with the Assistant Director and Director with the importance of reporting any incident that is not part of the usual occurrences of the day. If there is any question that comes up then they should check with the Child Care Licensing Department and consult with the on-duty analyst if the incident is reportable. LPA printed the 101212 child care regulation for Reporting Requirements and referred them to the Unusual Incident Report (UIR) LIC 627. The following deficiency listed on the attached LIC 809 (deficiency page) are being cited in accordance with California Code of Regulations Title 22. Deficiency that are being cited need to be cleared to protect the children’s health & safety. A notice of site visit was given and must remain posted for 30 days.
