What the inspector wrote

On 10/21/25, Licensing Program Analysts (LPAs) Diana Ortiz and Roxanna Lopez conducted an unannounced Case Management-Incident Report visit. LPAs met with Director Adetayo Akiwowo who guided LPAs on a tour of the facility. LPA observed 28 children and 8 staff present during this visit. The department received an unusual incident report on 10/23/25 via email for an incident that occurred on 10/23/25. Per Title 22 Regulations the report was reported in a timely manner. During today’s visit, LPAs conducted interviews with one child, staff, and Director. LPAs observed the outdoor play yard where the incident occurred. LPAs toured the facility and the outside play yard. Director explained the incident occurred after lunch time. S#1 stated the incident happened after lunch. S#1 stated she observed C#1 fall down from the play structure steps on to the mushroom which is also a part of the play structure, and hit his head. S#1 stated she was about 2 feet from where C#1 was playing. S#1 stated when C#1 fell, she rushed to him right away and noticed he was bleeding. S#1 then applied paper towels on C#1 forehead. S#1 stated S#2 then rushed to the C#1 and began assisting in providing first aide. S#2 stated the wound was rinsed with water and then she applied an ice pack on the wound. S#1 and S#2 stated the mother was called right away and C#1 was picked up within a few minutes. The parent then took C#1 to the Emergency room where he received one stich on his forehead. Per Director, C#1 returned to school the next day and parent was provided with an incident report from the facility. After interviews, observation of the areas and documentation of the injury, it was determined that staff were unable to prevent the injury from happening; therefore, there is no preponderance of evidence to determine that a regulatory violation occurred, the incident was determined to be an accident. Page 1 of 2 No deficiencies or civil penalties are being cited at this time. Exit interview conducted and report reviewed with Director Adetayo Akiwowo. A notice of site visit was provided, and appeal rights were provided and discussed. Page 2 of 2
