What the inspector wrote

On 4/15/2025 at 9:35 AM Licensing Program Analyst (LPA) Claudia Kam conducted an Unannounced Case Management inspection to follow up on an incident that was reported to The Department on 4/9/2025. Upon arrival, LPA met with Facility Representative (FR) Principal Ernestine Lara. Census was taken. On 4/9/2025, an incident was reported to The Department in regards to personal rights. The facility reported this incident to the Department within the required 24 hours and submitted a written report via email within seven days, complying with Title 22 Regulation requirements. During this inspection LPA interviewed FR and staff, child involved in incident were not present at the time of inspection. FR followed LAUSD protocols regarding these types of incidents, reporting to East LA Sheriff Department, reporting Operations Coordinator and writing an ISTAR and SCAR report. Facilty Representative was instructed by East LA Sheriff Dept that no further action needed. Facility Representative states that she interviewed all involved parties which includes interviews of parent and children. FR states that child returned to care the next day and continues to attend. FR states that no medical care was required for incident and children's Authorized Representatives were notified. Due to insufficient information being available at this time, a follow up visit will be required at a later date. Based on this information, the following deficiencies on the attached LIC 809D are being cited in accordance with California Code of Regulations Title 22. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the principle Ernestine Lara.
