On 10/28/2025 at 1:25 PM Licensing Program Analysts (LPA) Claudia Kam conducted an Unannounced Case Management inspection to follow up on an incident that was reported to The Department on 9/12/2025. Upon arrival, LPA met with Facility Representative (FR) Principle Viviana Ledeza. An incident was reported to The Department in regards to personal rights. The facility reported this incident to the Department within the required 24 hours and submitted a written report via email within seven days, complying with Title 22 Regulation requirements. During this inspection, LPA interviewed staff and children. Based on interviews the allegations have been substantiated. FR followed LAUSD protocols regarding these types of incidents, reporting to LAPD Child Abuse Unit, reporting Operations Coordinator and writing an ISTAR and SCAR report. FR was instructed by LAPD to handle administratively. FR states that they conducted their own internal investigations regarding the reported incidents which includes interviews of parents, children and staff. The internal investigation is concluded and is substantiated and pending operations. LPA interviews confirmed the incident and that a child's personal rights have been violated. S1 confirmed that she witnessed S2 force a child to sleep and that the child was injured during the incident. C1 confirmed that he was injured while in care due to being forced to sleep and that he no longer wishes to come to school because of the experience. Based on this information the following deficiencies on the LIC 809 D are being cited today 10/28/2025. LPA Claudia Kam informed facility representative Viviana Ledezma that this report dated 10/28/2025 documents 1 Type A citation which shall be posted for 30 consecutive days as there is an immediate risk to the health, safety, or personal rights of children in care. Also, LPA Claudia Kam informed the facility representative to provide a copy of this licensing report dated 10/28/2025 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. Report Ends - Page 2 of 2
