Page 2: Unsubstantiated
Pertaining to the allegation: Children are not wearing masks. On this date, 7/28/2021, at 9:05 a.m., LPA observed 80 children out of a total of 81 children wearing a face mask inside the classroom. Per staff interviewed, corroborating statements were made that some parents bring their child a mask. If children do not bring a mask, staff will provide an extra disposable mask for each child, along with making sure children wear the mask all day, giving verbal reminders to keep mask on. LPA observed extra disposable face masks in each room, and a COVID-19 ‘Face Mask Required’ sign posted on front entry gate. Per parents interviewed, multiple disclosures were made that COVID-19 protocols are to wear masks at the Preschool.
Pertaining to the allegation: Children are not practicing social distancing. On 6/23/2021, during nap time, LPA observed children sleeping in opposite directions, and observed napping equipment and children’s desks, distanced apart, in each classroom: Room #1, Room #2, Room #3, Room #5, Room #6, Room #7, and Room #8. Photos were taken. Per Director, there is no Room #4. Per staff interviewed, multiple disclosures were made that during breakfast, the teachers place the tables distanced apart, placing the chairs at each table on each side of the table, so the children are not sitting close to each other. Staff state there are 4 children per table.
During investigation, LPA conducted interviews with: Complainant, staff, and children’s authorized representatives. During visit to facility on 6/23/2021, LPA gathered the following documentation which includes: Staff schedule for month of June, Staff Time sheets, and Children's sign in and sign out sheets for month of June, Activity Schedules for each Preschool classroom, and a Facility Roster. LPA Jennifer Hua conducted a Rapid Assistance Support Team technical assistance video call via FaceTime to facility on 7/6/2020 regarding Department of Public Health Guidance for COVID-19 preventative measures. Per Director, receives Provider Information Notices (PINs) regarding continuous updates to Department of Public Health Guidance for COVID-19.
Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegations are Unsubstantiated.
Exit interview conducted with Director Hong Hou. A copy of this report (LIC 9099), and Appeal Rights were provided.
A "Notice of Site Visit" and copy of the report was issued. Notice of Site Visit must remain posted for 30 days. Failure to do so will result in a $100.00 civil penalty

Facility Roster. Per staff, school provides breakfast inside the “Breakfast Room” between 7:00 a.m. and 9:00 a.m., Staff also stated that one staff serves the breakfast, one staff watches the children, and one staff takes children’s temperatures and escorts the children to their assigned classrooms. Multiple staff disclosures were made that different children, from different classrooms, eat together in the “Breakfast Room,” and have always scheduled breakfast this way. Multiple children authorized representative disclosures were made stating due to COVID-19, children were dropped off at the front entry gate and a staff member would escort their child to either the “Breakfast Room” or child’s classroom.
LPA Jennifer Hua conducted a Rapid Assistance Support Team technical assistance video call via FaceTime to facility on 7/6/2020 regarding Department of Public Health Guidance for COVID-19 preventative measures, providing technical assistance to the facility. Per Director, Owner receives Provider Information Notices (PINs) regarding continuous updates to Department of Public Health Guidance for COVID-19.
On 6/23/2021, the Director did not ensure the personal rights of children in care in regards to safe and healthful accommodations, as required by the California Department of Public Health Guidance, issued June 18, 2020 and updated May 22nd, 2021, in that, staff did not separate the stable groups, mixing the cohorts of children, during the COVID-19 pandemic,
Based on LPA’s observations, interviews conducted, staff’s own admittance, record review, and documents obtained, the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED. California Code of Regulations, (Title 22, Division 12 & Chapter 1 Article 6), are being cited on the attached LIC 9099D.
During today's visit, LPA Gontarek assisted Director in setting up emails to receive Provider Information Notices (PINs) and assisted in navigating through LA County Department of Public Health's website for guidances.
Exit interview conducted with the Director, Hong Hou. A copy of The Complaint Investigation Report (LIC 9099 & Appeal Rights were discussed and provided at the time of the visit. A "Notice of Site Visit" and copy of the report was issued. Notice of Site Visit must remain posted for 30 days. Failure to do so will result in a $100.00 civil penalty.
