Page 1:
On 12/16/2025 at 9:35 am, Licensing Program Analysts (LPAs), Crystal Green and Diana Ortiz conducted an unannounced Case Management inspection for an increase in capacity. Upon arrival, LPA met with Facility Representative Carolina Villa- Crelenciame. Licensee, Primanti Montessori School- Whittier, Inc., has requested to be licensed for infants from ages 6 weeks to 24 months in classrooms Infant Room 1 and Nursey Room. There is also a Preschool Program (198016235), School Age Program (198021305), and Private Elementary School located on the same premises. Hours of operation will be Monday to Friday from 6am to 6pm. Present during today\342\200\231s inspection were 14 infant children with 4 staff. Appropriate ratios and supervision were observed. All indoor and outdoor activity space utilized for the children was inspected today. LPAs informed Licensee Representative that staff are required to maintain direct visual supervision of the children at all times during indoor and outdoor activities.
LPAs continued to tour the facility and measured the existing areas and new additional indoor activity space. Total indoor activity space measured 1163.30 square feet, which is sufficient to accommodate the requested capacity of 30 Children. LPAs observed all indoor activity space to be complete with safe, age-appropriate furniture and equipment, including tables, chairs, cubbies, napping cots, bookshelves, and other activity supplies for the children. Drinking water is available in the classrooms via water faucet attached to sink.
Highchairs and Feeding tables have broad-based legs, plastic seats are in good repair and trays lock onto chairs. Highchairs are made of washable, moisture-resistant material. Sides of the changing table are at least 3\342\200\235 high and the changing table is within arm\342\200\231s reach of a sink. LPAs observed the Infants to have (2) changing tables available within arms reach of a sink. Diapering sink is not used for meal preparation or dishwashing.

Cribs meet regulatory requirements. Crib area is separated from activity space by being located in separate room within Infant Room 1. LPAs observed 8 cribs available in the crib area. Sleep logs are kept in the crib area. Needs and Services Plans and Sleep Plans are kept in the Director's office.
LPAs observed a swimming pool located on the opposite side of the facility (school age program). There is a gate separating the school-age yard that gives access to the swimming pool. LPAs observed the swimming pool gate to be in compliance with Title 22. LPAs observed a life ring and a rescue pole. LPA did not observe an alarm in the pool. The entrance gate swings away from the pool and has a self-latching gate with a lock and key. Per Facility Representative, the pool is in need of repairs and is currently not in use. They have plans to drain and repair the pool during their holiday break, which is the end of December at the beginning of January. LPAs observed all hazardous items to be inaccessible to children. Fire clearance was granted on 10/23/2025.
There are (2) potty chairs, (3) sinks, and (3) toilets available for infant children\342\200\231s use. These are sufficient to accommodate the requested capacity of 30 children. There is a separate staff restroom equipped with a toilet and a sink. The isolation area for children who are ill will be the Director\342\200\231s office and ill children will use the staff restroom.
The facility currently has a fully fenced playground area. Fencing is made of concrete walls and wrought iron which is at least four feet high. The total square footage for all the outdoor activity space is 2491.92 square feet, which is sufficient to accommodate the requested capacity. Shade is provided via fixed shade structure and shade trees. There are sufficient outdoor age-appropriate toys and play equipment available on the playground. Drinking water is available via filter water jug and child\342\200\231s personal water bottle. LPAs observed all hazardous items on the playground to be inaccessible to children. Facility Representative was reminded that any changes to the facility must be reported to and approved by Community Care Licensing.

Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For childcare center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP). Per Facility Representative, the building was completed in 2011, and lead testing was last completed on September 13, 2017.
LPAs reminded the licensee of the safe sleep regulations and discussed the Child Care Licensing Safe Sleep webpage at:

, and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.
Facility representative was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated.

The following correction is needed prior to the issuance of the license:
The licensee must obtain and submit proof of the new Infant Room 2 complete with age appropriate tables and play equipment.
Licensee Representative, Carolina Villa- Crelenciame, understands that all proof of corrections must be provided to the Department within 30 days, or the application may be denied.
Once proof of correction is received, a license for 30 children from ages 6 weeks to 24 months will be granted upon final review.
Exit interview conducted and report was reviewed with the Facility Representative, Carolina Villa- Crelenciame.
A notice of site visit was given to facility representative and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days.
