What the inspector wrote

Licensing Program Analyst (LPA) Thelma Razo and LPA Lilli Babcock conducted a subsequent unannounced Required - 1 Year inspection. LPAs held this visit due to Maryvale provides services to military families. According to Director Stephanie Garcia, licensee has no child of military families enrolled at this time. Maryvale also has an infant/toddler program LIC # 198016250 and school age program LIC #198016249 . Waiver on school age license to allow preschool and school age programs to use entire yard on staggered schedule. On 10/6/2021, LPA Razo conducted a facility tour to include 6 classrooms, outdoor playground, bathrooms, kitchen and lead teacher office where medications are centrally stored. Today, LPAs reviewed children, staff and facility records. Per Director, fire and earthquake drill is being held every month (logs provided). All individuals present have obtained a criminal record clearance or criminal record exemption. There is at least one person trained in CPR and Pediatric First Aid present during this inspection . The review of Staff records was documented on the LIC 859. Staff present have proof of the AB 1207 Mandated Reporter Training certificate on file and proof against TB, measles, pertussis, and influenza. All staff have been given on the job training on sanitation principles, housekeeping, including universal health precautions. The Notice of Site Visit must remain posted for 30 days during the hours of operation after each site visit by a licensing representative.
