What the inspector wrote

On 12/10/2025, Licensing Program Analyst (LPA) Stephanie Li conducted a Case Management visit for the purpose of a Default Decision and Order received for one of the facilities temp teachers previously associated to the facility. LPA met with Director Stephanie Garcia. On 12/4/25, a default decision and order was issued with effective date 12/15/2025 for Temp 1. LPA contacted facility and spoke to the interim director Letica Larios on 12/08/25. LPA inquired if they had received the default decision order. Per interim, they did not receive it. During today’s visit, Director Stephanie confirmed that Default decision order issued on 12/4/25 has not been received. LPA emailed the default decision order to Directors email address provided. Director was aware of the issue and stated she has received many calls over the past year regarding the individual. Per director, temp 1 has never worked at the Duarte Maryvale and was only a sub for the Rosemead Maryvale during April-May 2019. Per director, the temp was provided through Child Care Careers agency and she has requested that they block him from all of their sites. Director provided email copies of correspondence with Child Care careers and previous LPA’s regarding the individual. LPA obtained current LIC500 personnel roster during visit and Child Care Careers contact info. At this time, the facility is in compliance with California Code of Regulations Title 22. No deficiencies are being cited at this time. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the Director Stephanie Garcia.
