What the inspector wrote

Licensing Program Analysts, Seung Lee and Shushan Safaryan conducted an unannounced annual random inspection to the licensed home. Upon arrival, LPAs met with Licensee Shelly Wong and toured the facility. There were 4 children present, along with Licensee's husband and 1 assistant. Individuals residing in the home are the licensee, spouse, mother, and two adult children. Licensee\342\200\231s operating hours are Sunday- Saturday, 7am-10pm. The home is a one story, 4-Bed, 3-Bath home. The following areas are used for day-care: Living room, play/nap room, dining room, 1 bedroom (for napping infants), 1 restroom, and backyard. Off limit areas include: Section of the home where bedrooms and remaining restrooms are located. Only one bedroom is used for napping infants which licensee escorts them to. Off limits areas have safety gates. Licensee has the required postings in the living room on the wall. Licensee's First Aid/CPR certificate are valid through 01/2025, Licensee's disaster drill log notes last drill conducted on 8/13/23. Licensee has a working telephone. Children's files and roster were reviewed. LPAs inspected all areas used by the daycare children. LPA observed appropriate toys, games, and books for children. Children's restroom was free of hazards and in clean condition. The kitchen has latches on cabinets that store knives and cleaning compounds. LPA observed the carbon and smoke detectors in the home and found them to be operational. The fire extinguisher, located in the dining room was purchased on 04/23/23. There are no firearms present on the premises as stated by licensee. There are no other pools or spas, or other bodies of water. There are two dogs that reside in the off limits part of the home. There are adequate age appropriate toys, books, and games. There are no firearms present on the premises as stated by the assistant. There were no pools or spas, or other bodies of water observed. There is one dog that resides in the off limits part of the home per Licensee. During file review, LPAs observed that an assistant who was present in the home (Staff#1) was not associated to this facility number. LPAs did verify that Staff#1 does have a valid criminal record clearance. The Licensee stated Staff#1 attempted to submitted new finger prints but was not sure what happened. Based on the file review and information verified, Staff#1 did not request a transfer of finger prints and instead attempted to submit a new live scan but was not associated due to some error. This is an immediate risk to children in care. The following were discussed : Individuals who are 18 years of age or older living in the home must be finger print cleared prior to being in the presence of the children in care. Individuals within one month of their 18th birthday must be fingerprinted immediately. No smoking, No infant walkers, No baby bouncers, No Johnny jumpers, No exersaucers and any other item that falls into that category. LPA discussed disaster drills, posting requirements, children records requirements, mandated child abuse and injury/death reporting. During file review, LPA Lee observed that the two assistants had current CPR, mandated reporter training certificate, and proof of immunization available.LPA reviewed SIDs, Never Shake A Baby, and safe sleeping practices. Infants should sleep mouth up, on their backs, free of clutter surrounding their sleeping space. Incidental Medical Services are not provided at this time. Licensee was informed to contact the department 30 days prior to enrolling any child with IMS needs. LPA advised licensee to refer to Section 101173 and 101226 for further information on regulatory requirements. Deficiencies cited during this inspection is available to view on the attached 809D page. The notice of site inspection along with all pages of this report must remain posted for a period of 30 days during business hours. Failure to maintain posting will result in a civil penalty of $100.00 dollars. LPAs provided Licensee with a LIC 9224. Licensee was advised to obtain signatures from all current parents and future parents for 1 year starting from this date. Exit interview conducted with Licensee Shelly Wong. Appeal rights discussed and explained.
