What the inspector wrote

On 04/22/2026 at 7:35am, Licensing Program Analyst (LPA) Jonnisha Culbert conducted an unannounced  inspection at the facility noted above. LPA met with licensee, Eresh Ratnayake, stated the purpose of today's inspection and provided them with an Entrance Checklist for Family Child Care Homes. Per licensee, operating hours are Monday through Friday from 9am to 6pm. Individuals residing in the home were discussed and noted. Present during the time of today's inspection were licensee, adult 1, and seven children. At approximately 9:10am., licensee Piseiy Kram arrived at the facility. This is a single family home that consist of three bedrooms, two bathrooms, living room, kitchen, enclosed patio (children's room), office, attached garage, front yard, and fenced backyard. Areas that are off-limits to children are: two bedrooms, one bathroom, kitchen, living room, office, and attached garage. The following documents were posted: Facility License, Notification of Parents' Rights (PUB 394), Earthquake Preparedness (LIC 9148), and Emergency Disaster Plan (LIC 610A). Areas used by children were inspected for safety, comfort, heating, cleanliness, and telephone service. LPA observed age-appropriate toys and furniture (toddler tables and chairs).  Per Licensee, the home is equipped with central heating and air.  Per licensee medications are kept in one of the off limits rooms. LPA reminded Page 2 licensee that all off limits areas of the home are to remain inaccessible to children. LPA observed Wasp and hornet spray in an accessible cabinet in the backyard. This is a potential health and safety risk to persons in care. Licensee removed the spray and placed it in one of the off limits areas of the home. Licensee was reminded that poisons are to remain in areas that are locked and inaccessible to children.   Per licensee, there are no firearms or bodies of water on the premises. LPA checked the home for required emergency devices. LPA observed a fire extinguisher (3-A:40-B:C) that is fully charged. LPA observed that the extinguisher was last serviced on 10/23/2025. Licensee was reminded to purchase or service the fire extinguisher yearly. Carbon monoxide detector and smoke alarm were observed and are operable. The facility roster and disaster drill log was reviewed and the last drill was conducted on 03/10/2026. LPA observed that child 1 and child 2's names were not on the facility roster. LPA reminded licensee to update facility roster. Per licensee, they do not provide meals and snacks for children in care. Licensee was reminded that food that is brought from the children's home shall be labeled with the child’s name and properly stored or refrigerated. LPA conducted a file review of two children's files. During record review, LPA observed that the children files were complete. LPA conducted a file review of the licensees files. During review, LPA observed that both Licensees have valid Pediatric First Aid/CPR/AED certificates, the required immunizations, and orientation certificates. Licensee, Piseiy was missing a valid mandated reporter training. This is a potential safety risk to persons in care. LPA reminded licensee that mandated reporter training must be completed every 2 years. Mandated reporter training is available at www.mandatedreporterca.com . Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. Page 3 LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02- CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. During the exit interview, Piseiy Kram, confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS.
