What the inspector wrote

On 05/18/2026 Licensing Program Analyst (LPA) Mary Silva conducted an unannounced case management inspection to ensure the facility was in compliance with Title 22 regulations. At the time of arrival licensing staff met with director Nicole Santos. The purpose of the inspection was explained. Census was taken. LPA MSilva observed a total of 42 children present with 5 teachers and 2 assistants. On 05/13/2026 the department was made aware of an incident that occurred at the facility on 05/12/2026 involving a child that sustained an injury in the Panda Classroom. During the inspection LPA conducted interviews with facility director and teacher that witnessed the incident. LPA obtained documentation in the form of facility roster, personnel report, incident report, status report from medical facility, picture of the injury and other documentation pertaining to the incident. Per interviews conducted, child #1 was running in the classroom during breakfast time. Staff#1 was sitting in the kidney table and observed child #1 tripped, fall and bumped right cheek bone into the bottom right leg of the wooden sensory table. Child #1 sustained a bruise on the right cheek bone as a result of the fall. The facility applied an ice pack and called parent. Per director parent took child to seek medical attention and returned to the facility on 05/13/2026 with no medical restrictions. Based on interviews conducted and the information verified during the inspection of the incident. Licensing staff did not observe any type of deficiencies that warranted a citation. Per director the sensory table was moved to the east side of the classroom against the wall. At this time, the facility is in compliance with California Title 22 Regulations. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Appeal Rights were given. Exit interview conducted and report was reviewed with Director Nicole Santos.
