What the inspector wrote

On 06/12/2025, Licensing Program Analyst (LPA) Kruz Long conducted an unannounced case management inspection. A COVID-19 risk assessment was conducted. LPA met with Florence Banda, Director and explained the purpose of the visit. There are 16 children with 6 staff members present during today's visit. The purpose of the visit is to follow up on an incident that occurred on 06/05/2025 and was reported to the department on 06/06/2025. The self reported incident is regarding supervision. During today's inspection, LPA interviewed Staff #1 (S1) to Staff #3 (S3) in the office, obtained a copy of the children roster and disciplinary documents. LPA toured the toddler and infant classroom with the Director. Children not interviewed due to age. There are no deficiencies being cited today as the incident requires further investigation. An exit interview was conducted and a copy of this report and appeal rights was provided to the Director. A Notice of Site Visit was provided; Notice of Site Visit must be posted for 30 days.
