What the inspector wrote

On 01/30/2026, Licensing Program Analysts (LPAs) Mary Silva and Caroline Nguyen conducted an unannounced case management inspection. LPAs met with Heather Jenkins, Regional Director and explained the purpose of the visit. Census was taken. LPAs observed 7 infants with 2 staff in classroom #1 and 6 infants with 2 staff in classroom #2. LPAs were led on a tour of the facility. The purpose of today’s visit is to investigate an incident that occurred on 01/08/2026 and was reported to the department on 01/09/2026. The self-reported incident is regarding care and supervision. During the course of the investigation, LPA(s) interviewed Regional Director and 3 staff, reviewed file for staff #1 and child involved in the incident, reviewed video footage from the date of incident 01/08/26 from 4:49pm-5:59pm, photograph was taken of classroom #1, obtained a copy of the facility roster, sleep log, personnel report, sign in and out sheet from 01/08/26, “Name to Face” report from 01/05/26-01/16/26, infant sleeping policies, classroom expectations and other documents pertaining to the investigation. According to interviews conducted, staff #1 disclosed that on 01/08/26, child #1 was signed out on App Tadpole between 5:25pm-5:30pm, per staff #1 was not aware child #1 was napping in the crib which is located in the rear side of the napping area. Based on video footage staff #1 left classroom at 5:25pm to the front office area. When parent arrived at the facility at approximately 5:38pm via Procare App, staff #1 realized child #1 was in the classroom. At 5:39pm staff #1 was viewed returning to classroom #1. According to the video footage child was left alone and unsupervised for approximately 14 minutes from 5:25pm-5:39pm. Based on interviews conducted and video footage reviewed, the facility did not provide care and supervision to the children in care which poses an immediate risk. Page 1 of 2 The following deficiency listed on the attached LIC 809D (deficiency page) are being cited in accordance with California Code of Regulations Title 22. Deficiencies that are being cited need to be cleared to protect the children’s health and safety. LPAs informed licensee Heather Jenkins that this report dated 01/30/26 documents 1 type A citation which shall be posted for 30 consecutive days as there is immediate risk to the health and safety or personal rights of the children in care. Also LPAs informed Heather Jenkins to provide a copy of this licensing report dated 01/30/2026 that documents any Type A citations to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parent/guardian for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement must be placed in the child’s file for verification. Licensee shall post report documenting Type A citation along with Notice of Site Visit Form in an area accessible for review for 30 days or a civil penalty of $100 will be assessed.     (a) The licensee shall provide care and supervision as necessary to meet the children's needs. (1) No child(ren) shall be left without the supervision of a teacher at any time, except as specified (b) in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation. This requirement is not met as evidence by: Per Staff #1 and video footage staff #1 left classroom at 5:25pm to the front office area and returned to classroom #1 at 5:39pm. When parent arrived at the facility, staff #1 realized child #1 was in the classroom.
