What the inspector wrote

On 04/30/2026 a meeting was held at Pomona Unified School District-Resource and Referral Agency as a follow up to the Noncompliance Conference called on 03/25/2026. Present during this meeting are Regional Manager Deborah Ajao, Licensing Program Manager Christina Gabelman, Licensing Program Analyst Mary Silva, Licensee Beth Holmes, and Licensee David Holmes. This meeting was called to discuss the following issues or deficiencies. On 04/06/2026, facility self-reported an incident to the department regarding a child that sustained an injury in the toddler classroom. · Type A citation issued for personal rights on 04/14/2026: Case Management-Incident Inspection: On 04/06/26 Child #1 (1 year 10 month old) sustained a fracture to right leg while climbing a toddler size bookshelf according to video footage and interviews conducted. Facility implemented the following corrections to prevent similar incidents from occurring: · Bookshelf was removed from the toddler room. · Staffing was changed from one teacher and two assistants to two teachers and one assistant. Due to the history and nature of violations including the incident that occurred on 04/06/2026 the department is taking the following action. Licensee of Kiddie Academy of Glendora (Fac #198016423), (Fac #198016424), (Fac #198016425), was advised that the case has been referred to the Department's Legal Division for revocation. Regional Manager, Deborah Ajao, explained the Administrative Action process. Licensee was advised that any additional serious violation of the regulations may result in a more immediate administrative action being taken against the licensee. Exit interview was conducted, appeal rights and report was reviewed with licensee, Beth Holmes.
