Licensing Program Analyst (LPA) Nolan Tcheng conducted an unannounced Case Management-Other for the purpose of providing an Advisory Note-Technical Violation to the facility. Upon arrival at 11:08am, LPA was met by Director Erinn Levin, to whom the purpose of the inspection was explained. At 11:12am, LPA was led on a tour of the facility. Census was taken. During the time of inspection, LPA observed 2 staff and 11 children in the outdoor play yard. An annual inspection was conducted for the facility on 01/28/2022. Per Director, the staff files are kept at their Human Resources (HR) Office at the address 236 W. Mountain St. #102 Pasadena, CA 91103. On 02/02/022, LPA conducted a visit to the HR Office to conducted staff file review for the three staff members present during the Annual Inspection on 01/28/2022. During the staff file review, LPA observed that Staff #3 did not have an up to date Mandated Reported AB1207 Child Care Provider Training completed. The certificate on file expired 02/2021. LPA is providing an Advisory Note-Technical Violation, to address the expired Mandated Reporter Training. No deficiencies are being cited during this inspection. The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Exit interview was conducted with Director Erinn Levin, at 11:55am, including a copy of this report.
