On July 22, 2025, Licensing Program Analyst (LPA) Monique Ayala conducted an unannounced Case Management Incident inspection at the above facility. The purpose of the inspection is to follow up on an incident that occurred sometime in 2023 - 2024 and the facility was notified of the incident by a parent on 07/15/2025. The facility informed the department of the incident on the same day the incident was brought to the directors attention on 07/15/2025. LPA met with director, Samantha Montes who guided LPA on a tour of the facility. LPA observed 19 children in care with 3 staff members. LPA interviewed Staff #1 (S1) to Staff #2 (S2). The director will forward LPA a copy of the 2023-2024 facility roster and current facility roster via email. LPA was unable to interview Child #1 (C1) and Child #2 (C2) as they no longer attend the facility. Based on interviews, S1 and S2 were not aware of any incidents that had occurred with C1 and C2. Per S2, the parent of C1 stated there was an incident that occurred with C1 and C2 over a year ago. S1 and S2 stated there was no uncommon incidents that had occurred between C1 and C2. There are no deficiencies being cited at this time as further investigation is needed. An exit interview was conducted and a copy of this report was provided to the director along with Appeal Rights. A Notice of Site Visit was provided and must be posted for 30 days.
