Incident investigation·March 30, 2026

On March 30, 2026, Licensing Program Analysts (LPAs) Monique Ayala and Seung Lee conducted an unannounced Case Management - incident inspection at the above facility. LPAs were greeted by director, Samantha Montes who guided LPAs on a tour of the facility. LPAs observed 15 children with 3 staff members. The purpose of the inspection is to follow up on an incident that was reported to LPA Ayala in person on 03/30/2026. The director was made aware of the incident on 03/27/2026. The incident reported is that the facility currently does not having running water as their plumbing needs to be repaired. Per director, they were made aware of the plumbing issue the evening of 03/27/2026. Per director they requested from the building owner a mobile restroom that complies with Title 22. The mobile restroom has a boys side with 1 stall, 1 urinal and 1 sink; the girls side has 2 stalls and 1 sink. LPAs observed the restrooms to flush and the sinks to have running water. Per director they are unaware of how long it will take the plumbing to be fixed as the incident occurred on over the weekend. LPAs observed the mobile restrooms to be placed/located outside the facility gate/door in the parking lot. Director placed a temporary barrier around the mobile restroom to ensure children do go into the parking lot. LPA Ayala advised that a more secure barrier is placed. LPAs informed director that there needs to be a more secure barrier around the mobile restrooms, staff need to accompany children to the restroom at all times and director needs to provide LPA Ayala with a time frame of when the plumbing is going to be fixed as the mobile restroom is temporary. LPAs obtained images of mobile restroom. There are no deficiencies being cited at this time. An exit interview was conducted and a copy of this report was provided. A Notice of Site Visit was provided and must be posted for 30 days.
