Incident investigation·April 30, 2026

On April 30, 2026, Licensing Program Analyst (LPA) Monique Ayala conducted an unannounced Case Management- Incident inspection. LPA met with director, Samantha Montes who guided LPA on a tour of the facility. LPA observed 22 children in care with 3 staff members. The purpose of the inspection is to follow up on an incident reported that was reported to the department on 04/13/2026; the incident occurred on 04/09/2026. During the inspection LPA interviewed Staff #1 (S1) to Staff #3 (S3). There are no deficiencies being cited at this time as the incident requires further investigation. An exit interview was conducted and a copy of this report was provided to director, Samantha Montes. A Notice of Site Visit was provided and must be posted for 30 days.
