On March 10, 2025 @ 10:20am Licensing Program Analysts (LPAs) Lilli Babcock and Monica Ruiz conducted an unannounced annual inspection to the above facility. A COVID risk assessment was conducted. LPAs met with Licensee, Rehem Behiri-Lopez, to whom the reason for the visit was explained. LPAs observed 2 children and one assistant present at the time of our arrival. Per Licensee, there are 12 children currently enrolled. There were 2 children present upon arrival, one child was dropped off shortly after we arrived. Names and ages of all children present at time of inspection, were recorded. The licensee was observed to be operating within the license capacity limitations at the time of inspection. The licensee states that four adults, and one child currently live in the home. Per Licensee, there is one assistant employed by the facility. All adults living in the home have obtained a criminal record clearance. This is a two-story home which consists of 3 bedrooms and 2 bathrooms upstairs, and downstairs contains 1 bedroom,1 bathroom, living room, dining room, family room, kitchen, and daycare room in the converted garage. Facility also contains a front yard and backyard which is fenced and divided into 2 areas (one for daycare to use and one that is off-limits). ----------------------------------page 1of 5--------------------------------------------------------------------------------- Licensee guided analysts on a tour of the facility. During this visit, all areas identified on the facility sketch that are accessible for children to use were toured and inspected and the following was observed. Per licensee, areas off-limits to children and parents include the entire second floor, and 1 bedroom, dining room, family room, front yard and half of the fenced backyard downstairs. The children have access to kitchen, living room, one bathroom, daycare room in converted garage and the portion of backyard that is fenced off next to the dining room and living room. Facility License, Emergency Disaster Plan, Parent\342\200\231s Rights Poster, and Earthquake Preparedness Checklist were observed to be posted next to the door leading to the outside of the home. A current children\342\200\231s roster was available for review. There is telephone service via a cellphone that stays at the facility during operation hours. Hours of operation are Monday through Friday, 7:00 AM to 5:30 PM. Licensee cares for children ages 6 weeks to 12 years old. Detergents, cleaning compounds, and medications were observed to be inaccessible to children. Per Licensee, there are no poisons on the premises, and none were observed on this day. LPA advised that any poisons should be locked under key or combination lock. Licensee states that there are no firearms or weapons stored in the home, and none were observed on this day. The valve on the required 2A 10BC fire extinguisher indicates fully charged and was serviced on 7/15/2024, as indicated on the service tag. Smoke and carbon monoxide detectors were tested and were operable on this day. All homes shall conduct fire and disaster drills at least once every six months and document the date and time of each drill. Last fire drill conducted was 3/3/2025. ------------------------page 2 of 5 ------------------------------------------------------------------------------------- The home is observed to be clean and orderly. There are first aid supplies available. There are age-appropriate toys available for children, such as a playhouse, play kitchen, books and sensory toys. Appropriate sleeping arrangements in form of playpen and cots were observed. Licensee states she is not caring for infants currently. Currently, children are using the back yard for outdoor play time. The outdoor play area was observed to be fenced. LPA observed that the outdoor yard has toys such as a Kid Kraft Playhouse and other materials for children to play with. Children\342\200\231s records were reviewed, including emergency information, and were observed to be complete. Licensee records were reviewed and observed to be complete. LPAs observed that the Licensee does have proof of pertussis, measles and influenza immunizations, and the Mandated Reporter AB 1207 compliant Child Care Training Certificate on file with expiration on 11/14/2026. The licensee has completed training on preventive health practices including Pediatric First Aid and CPR. The licensee's Pediatric First Aid and CPR expires on 10/2/2025. LPAs observed Pediatric CPR card to have EMSA stickers on back. LPA issued an LIC 857, Children\342\200\231s Record Review, and LIC 859, Staff Record Review to the licensee which documents staff and children\342\200\231s files reviewed during this inspection. There are 2 dogs on the premises, one large Sheep-a-doodle and Golden-doodle. Per licensee, dogs are inaccessible to children in care and has immunization records. Smoking is prohibited in a licensed Family Child Care Home. Per Licensee, no one smokes in the home. ---------------------------------------------------page 3 of 5----------------------------------------------------------- LPAs did not observe the following items during the inspection: Johnny Jumpers, Saucer Chairs, and/or any other item that fall into these categories are not permitted in a family child care facility. Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. -------------------------------------------page 4 of 5------------------------------------------------------------------------ Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02- CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. During the exit interview, the Licensee Reham Behiri-Lopez, confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS. At this time, the licensee is in compliance with California Code of Regulations Title 22. No deficiencies cited. A notice of site visit was given and must remain posted for 30 days.
