What the inspector wrote

During interview with complainant- it was disclosed that Child # 1 was sent home with dirty diapers approximately 5 times, parents or authorized representatives were denied entrance into the facility, scrape on chin was not reported and Child # 1 disclosed being hit- however they were not able to communicate if they were hit by a staff member or a child. Child was not available for interview. In regards to allegation # 1- Staff did not change the child timely # 2 -Staff did not allow parents inside the day care ensure reporting requirements were followed # 3 Staff did not ensure reporting requirements were followed and # 4 Staff hurt day care child, LPA conducted interviews with complainant, staff, children and parents. LPA conducted interviews with staff # 1- 7. Regarding allegation # 1: Staff statements corroborated that children\342\200\231s diapers are changed every 1.5 hours \342\200\223 2 hours or as needed, diapers are changed before child goes home, if child is picked up before diaper they communicate with parents and change diaper if requested by parents. Regarding allegation # 2: All staff corroborated that they have not denied parents or authorize representatives entry to the facility or classroom. Regarding allegation # 3 Staff 1-6 corroborated that the process to report incidents is writing up a report- depending on severity of incident parents will be called at that time- once child is picked up report is signed by parents or authorized representative and a copy is given to parents and original stays in the facility. Staff # 3, 5 and 6 disclosed that they were aware of the bruise and scrape on chin. Per staff # 3, 5 and 6 the day before parent inquired about bruised/ scraped chin child # 1 was pushed by child # 2 during play time. Staff checked child # 1 and only observed a scrape on the knee- incident report was written on that day and parent was informed. Staff # 3, 5 and 6 corroborated that bruised scraped chin was not added to the incident report since it was not observed when child had incident. Regarding allegation # 4: Staff # 1-7 corroborated that they have not hurt a child in care or have observed any other teacher hurt a child in care. --------------------------------------------------------- pg. 2 of 3 -------------------------------------------------------- LPA conducted interviews with parents. Parent\342\200\231s statements corroborate that they do not have any concerns with the staff in the facility, they have been informed and received incident reports when their child has been hurt and have not been denied entry to the facility. LPA conducted interviews with children. Children\342\200\231s statements corroborated that they like their school and did not disclose being hurt by staff. This agency has investigated the complaint alleging Staff did not change child\342\200\231s diaper timely, staff did not allowed parents in the day care, staff did not ensure reporting requirements were followed and staff hurt day care child. Based upon the evidence as presented above, the allegation has been determined to be Unsubstantiated. A finding of Unsubstantiated means that although the allegation may have happened or is valid, there is not a preponderance of evidence to prove alleged violation(s) did or did not occur, therefore at this time the above allegation is unsubstantiated. A notice of site visit was given and must remain posted for 30 days.
