Follow-up on corrections·February 10, 2026

The purpose of this report is to amend the deficiencies listed as a type b citation as mentioned in previous deficiency report dated for 2/9/26. Director Rachel Cordova is in compliance with children's records under title 22 regulations Child has a care plan on file a copy was received by LPA Kamile Martin on 2/2/26. On Monday, February 2, 2026, a Case Management - visit was conducted for an Unusual Incident Report (UIR) received in the RO Regional Office on 1/28/26. Upon arrival, Licensing Program Analyst (LPA) Kamile Martin was greeted by Director Rachel Cordova who guided LPA Martin on a tour of the facility. Census was taken and LPA Martin observed 11 Children in care in the preschool classroom. Per Director, Rachel Cordova stated there are currently 19 children enrolled at the facility for preschool and 3 enrolled for infant program. During observations and video recording review, it was determined that personal rights were violated from 3 rd party Vendor employee which poses an immediate health, safety or personal rights risk to persons in care under section 101223 (3) personal rights. The deficiency listed on the following page was observed by LPA Martin and is being cited in accordance with California Code of Regulations Title 22. One type A deficiency is being issued today for regulation 101223 Personal Rights Please see attached LIC 809-D for citation. Deficiencies that are being cited need to be cleared to protect the children’s health & safety. A copy of this report shall be provided to the parent/guardian of children currently enrolled by the next business day or immediately upon their return. A copy of this report shall also be provided to the parent/guardian of any newly enrolled child for the next 12 months. A signed Acknowledgement of Receipt (LIC9224) shall be in each child’s file, acknowledging receipt.
