What the inspector wrote

On 09/05/2024 at 12:30 PM an unannounced Annual/Random Inspection was conducted by Licensing Program Analyst (LPA) Katrina Chicote. LPA announced purpose of visit and was granted entry to facility by Office Staff. Facility is currently licensed for a capacity of 24 and per record review have 11 enrolled. The program currently operates Monday thru Friday from 7:56 AM to 2:26 PM. LPA singularly toured facility. Required postings were observed. LIC 125 was provided. The facility is made up of one classroom, Room K-1, in Escutia Primary Center campus. At the initial start of the site visit, there is a total census of 11 children transitioning out of nap time and a total census of two staff (S1 and S2) present. Beginning at 12:35 PM, furniture and equipment were inspected for age appropriateness and good repair. LPA observed surface of classrooms to be safe and clean, and child size tables and chairs available for children with rounded edges and well maintained. LPA observed learning stations for dramatic play, math, and science with a variety of age appropriate toys maintained free of loose and sharp parts and a library area available for quiet play. Telephone service, heating, lighting and ventilation were evaluated. There were cubbies observed as storage for children's belongings. Mats were inspected. LPA observed children transitioning out of nap time and observed children's mats were laid out that did not hinder walkways and exits/entrances at time of inspection. Per FR linens are provided by the facility and washed by weekly laundry service. Availability of drinking water was reviewed. LPA observed children's personal water bottles labeled with their name stored in a mobile bin that is taken indoors and outdoors. Age appropriate sinks and toilets were inspected for availability, good repair, water temperature, toilet paper, area safety and sanitation. LPA observed two bathrooms available in classroom that each had one toilet and one sink available, totaling two toilets and three sinks (the third available in the classroom). Hazardous items including poisonous cleaning compounds were stored inaccessible to children. LPA observed these items stored in high cabinet at time of inspection that could be locked. Report Continues - Page 1 of 4 A first aid kit is kept in high teacher cabinet and was inventoried for necessary supplies . Per S1, she has observed a plug in carbon monoxide detector in the classroom previously but believes that carbon monoxide detector is now hard wired into smoke alarm system. LPA was not able to test or verify this information at time of inspection. LPA observed smoke detectors and fire extinguishers are present in the facility. Disaster drill log was available and last drill conducted last week. Isolation area for sick children is in school's Nurse's Office. Per S1, they currently have no children enrolled that require medication at time of inspection. Snack/lunch menus were reviewed and observed posted. Facility provides Breakfast and Lunch and PM snack is brought from home. LPA observed children bringing their lunch boxes, labeled with their names, out from their cubbies to eat PM snack at time of inspection. S1 states they do not prep food and just pick up food from near by Cafeteria. LPA observed trash cans with lids available used to discard food at time of inspection. Beginning at 12:43 PM, outdoor equipment was inspected for safety, cushioning material, good repair and age-appropriateness. LPA observed surface of outdoor play areas to be safe and maintained for activities planned. LPA observed a variety of age appropriate toys available such as dramatic play toys, sensory tables, and large outdoor manipulatives. Required shade and fencing were inspected. LPA observed pop up tents for shade and play equipment being used to barricade area that is used for license program to prevent co-mingling. LPA advised S1 that the children need to be within the direct care and supervision, including visual supervision of the teacher(s) at all times. The children have access to their personal water bottles during outside play. Play area was inspected for hazards and inaccessibility to bodies of water; no bodies of water or hazards observed at time of inspection. Teacher-child ratios were observed and staff names recorded. Care and supervision was evaluated to determine if the basic needs of children are met and appropriate. Sign-in and out sheets and procedures were reviewed. LPA observed all children present signed in at time of inspection. Personal Rights of children were observed by LPA at time of inspection. Beginning at 1:10 PM, staff and children\342\200\231s records were reviewed for completeness. Inspection of required forms was made and documented on the LIC 857 for children and the LIC 859 for staff. LPA did not review confidential staff files during the inspection due to the staff confidential files being stored at the administrative Report Continues - Page 2 of 4 office. CPR card expires on 08/2026 for S1. Staff present did not have proof of current AB1207 Mandated Reporter Training certificate on file at time of inspection. All staff and employees have obtained a criminal record clearance or criminal record exemption as a condition of employment with the Los Angeles Unified School District (LAUSD). The following were discussed: LPA advised of the requirement to report Unusual Incidents and/or injuries to the parent/guardian and to CCL within the time frame specified by the regulation, this includes any changes as soon as they occur such as construction, remodeling, telephone number changes and/or if you move from your location. LPA advised the licensee to access forms, regulations and quarterly updates on the Child Care Licensing website at: www.ccld.ca.gov . CCC COMPLETED SAMPLING BUT THE TEST RESULTS ARE NOT YET AVAILABLE : Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. LPA referred Facility Representative] to the Department website for lead: Lead Toxicity Prevention and Water Testing Information . Report Continues - Page 3 of 4 Incidental Medical Services (IMS) - Child Care Centers Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP . When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. The facility was found in compliance per Title 22 regulations, there will be no deficiencies cited only advisories given today, 09/05/2024. A notice of site visit was given and must remain posted for 30 days. Exit interview was conducted and report was reviewed with the Facility Representative, Noemy Morales, Principal.
