Licensing Program Analyst (LPA) Roxana Lopez conducted an unannounced Case Management inspection due to 1 incident that was reported to the Department on December 11th, 2025. LPA met with Principal Kathy Romo who guided LPA on a tour of the facility. Census was taken. On December 11th, 2025, an incident was self reported to the Department via Email by the facility who reported child #1 had fallen hitting their forehead. Child required medical attention. Incident was reported within the required 24 hours. The purpose of the inspection was to obtain additional information regarding the incident reported to the Department. During the inspection, LPA Lopez conducted interviews with staff and child # 1. Regarding incident staff statements corroborated that incident occurred and was directly observed by staff # 2. Per Staff # 2 they took 3 children to the bathroom including child # 1. When child # 1 was done washing hands they walked over to get a paper towel when they tripped on their own feet. Staff # 2 disclosed that when they saw child # 1 tripped they placed their arm on child # 1 to prevent them from falling but it was too late and child # 1 hit their forehead on the side panel of the bathroom door. First aid was provided and parents were called. LPA interviewed child # 1 who disclosed "my head- fell" and would point to their forehead. LPA asked child # 1 to show her where the incident happened. Child pointed to the area and the floor where incident that happened. -------------------------------------------------- pg. 1 of 2 ---------------------------------------------------- Per Principal and staff # 2 area where incident occurred was observed to be dry. Per Principal Child # 1 was taken to the doctor and received 5 stitches. Staff continues to monitor children in the bathroom and talk about safety with the children. At this time, the licensee is in compliance with California Code of Regulations Title 22. Therefore, no deficiencies are being cited. A notice of site visit was given and must remain posted for 30 days.
