What the inspector wrote

Licensing Program Analyst (LPA) Rita Ramos conducted an unannounced required 1 year inspection to the above facility on 02/18/22. LPA arrived at the facility at 8:39AM, identified self and met with Ola Bailey, Site Supervisor, who guided analyst on a tour of the facility. LPA provided Site Supervisor with a copy of the LIC 125 Entrance Checklist to help facilitate the inspection. This is a preschool program which consists of 4 small classrooms. During the COVID-19 pandemic, the facility chose to place half walls and plexi glass to ensure that small cohorts were maintained to reduce the risk or spread of the virus. What was once a large classroom is now divided into the 4 small classrooms. The facility has decided to keep the small classrooms in place. LPA advised the Site Supervisor to submit an updated facility sketch to reflect the change in the room to smaller rooms. COVID-19 precautionary measures were in place and observed. Facility operation hours are Monday to Friday from 6:30 AM to 6:00 PM. All areas identified on the Facility Sketch were inspected. Upon arrival, the following staff were present during this inspection: Room EB2B: Staff #1 with 9 preschoolers; Room E3CB: Staff #2 with 10 preschoolers. Staff #3 arrived during the tour. Rooms E3CA and E2BA did not have any staff or children during the tour. The facility was observed to be within the license capacity and limitations, however, LPA noted at approximately 8:56AM that Staff#1 and Staff #3 were not listed on the criminal record associations list. LPA called the department regional office at approximately 9:25AM to confirm if Staff #1 and Staff #3 are associated to the facility and it was confirmed that Staff #1 and Staff #2 are not associated to the facility. A civil penalty of $200 is being assessed during today's visit. The following was observed during the tour of the facility: Furniture and equipment were inspected for age appropriateness and good repair. Telephone service, heating, lighting and ventilation were evaluated. -----Page 1 of 3 Children have their own cubby to store their belongings. Linens are washed weekly at facility. Napping equipment (cots) were observed in a separate storage closet. Per Site Supervisor, the isolation area is located in the office. Age appropriate sinks and toilets were inspected for availability and good repair in all restrooms. General sanitation was observed. Availability of indoor drinking water was observed in classrooms. Disinfectants, cleaning solutions, medication and other items that are dangerous to children, were inaccessible to children. Site Supervisor states that there are no poisons stored at the facility. Carbon monoxide detectors were observed and tested to ensure that they are operable. All kitchen areas/food preparation areas and food storage areas are kept clean and are free of litter, rubbish, rodents, and/or any other vermin. All storage containers for solid waste, including moveable bins shall have tight-fitting covers that are kept on, and in good repair. Trash cans used to discard food have tight fitting lids. Outdoor playground equipment is in a safe condition, free of sharp, loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. All areas around or under high climbing equipment, slides, and similar equipment are cushioned with material that absorbs a fall. There is adequate shade in the play yard. Availability of outdoor drinking water was observed. LPA advised that no children shall be left without the supervision of a teacher at any time. All floors were observed to be clean and safe. All materials accessible to children were observed to be toxic-free There are no firearms stored on the premises. There are no pools or bodies of water at the facility. Site Supervisor was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated. There is at least one person trained in CPR and Pediatric First Aid present during this inspection. Children\342\200\231s Records were reviewed. Inspection of required forms was made and documented on the LIC 857. LPA also reviewed staff records. The review of Staff records was documented on the LIC 859. Upon reviewing staff files, LPA observed at approximately 9:57AM that Staff #4 did not have proof of a current AB 1207 Mandated Reporter Training certificate on file. ------Page 2 of 3 Staff present did have proof against TB, measles, pertussis, and influenza. Children's roster was reviewed and is current. Sign-In and Sign-Out sheets were reviewed. Children present were signed in. Disaster drill log was available, last drill was conducted on 01/10/22. Menus were observed to be posted. Incidental Medical Services (IMS) policy was discussed. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department.
