Inspector’s narrative

What the inspector wrote

Page 2 LPA advised for the gate door to be made inaccessible to children in care to prevent additional incidents. Based on interviews, although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is unsubstantiated. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and a copy of the report was provided to Site Supervisor Jessica Osorio.
