What the inspector wrote

On 04/21/2025 at 10:30 am , Licensing Program Analyst (LPA) Shushanik Safaryan and Licensing staff Youle Qi conducted an annual random inspection to the above facility. Licensing staff met with , Julie Leung, licensee`s assistant to whom the reason for the visit was explained. Per Assistant licensee`s is out to the store to purchase supplies for the day care. Upon arrival there were 11 children present with 6 assistants. The licensee was observed to be operating within the license capacity limitations. During this inspection individuals who reside in the home were discussed and notes on Confidential Name List (LIC811) and attached to this report. Per applicant operation hours are Monday to Friday 8:30 a.m. to 5:30 p.m. During this visit, all areas identified that are accessible for children to use were inspected for the safety, comfort, and cleanliness. This is a two-story home which consists of 4 bedrooms, 4 bathrooms, kitchen, dining room (classroom), living room (play room), family room(office area), attached garage, front yard and backyard (fenced). The children use front door bathroom, living room (play room), dining room (classroom), and backyard. Per licensee, areas off limits to children and parents are: 2nd floor, all bedrooms, 3 bathrooms, kitchen, attached garage, family room, and front yard. At 10:30 am , Assistant guided Licensing staff on the tour of the home . Licensing staff toured home starting from the main entrance. Per assistant, parents enter home through the main entrance. Children have access to the bathroom next to the entrance. LPA toured the bathroom and did not observe any hazards. Next LPA toured the living room and observed 11 cots . Per assistant this room used for napping . Next LPA toured the dining room where observed small tables and chairs. Page 1 of 4 LPA observed child safety gate from both sides of the kitchen to make kitchen inaccessible to the children. In the kitchen LPA observed bathroom used by staff and office area. In the office area , LPA observed 1 cat. Per assistant cat is always in the office inaccessible to the children . In the kitchen LPA observed 2A 10 BC required fire extinguisher with the receipt attached to it with the purchase date 08/17/2024. Next LPA observed smoke and carbon monoxide devices. LPA observed child safety gate blocking stairs to the second floor of the home. From the dining room area children use side door to exit to the backyard which is fenced . In the backyard in the patio area LPA observed 11 children with 4 staff members having lunch. In the gated grass area LPA observed playing equipment , swings and did not observe any hazards. LPA observed door to the attached garage closed during the day care hours. There is telephone service via a cellphone that stays at the facility during operation hours. There is central air and heating in the home. Detergents, cleaning compounds, and medications were observed to be inaccessible to children. Per Licensee, there are no poisons in the home. Per assistant there is a weapon in the home. All homes shall conduct fire and disaster drills at least once every six months and document the date and time of each drill. Last drill documented was conducted on 03/11/2025. The home is observed to be clean and orderly. There are first aid supplies available in the day care . There are age-appropriate toys available for children. Facility License, Emergency Disaster Plan, Parent\342\200\231s Rights Poster, and Earthquake Preparedness Checklist were observed to be posted in the front next to the entrance . A current children\342\200\231s roster was available for review. Per assistant 12 children enrolled , from whom 4 infants. Per licensee, she provides food for children in care. Licensee was advised that if food is brought from the children\342\200\231s homes, all containers must be labeled with child\342\200\231s name and properly stored or refrigerated. Page 2 of 4 At 11:00 am Licensee and staff records were reviewed. The licensee has completed training on preventive health practices including Pediatric First Aid and CPR. The licensee's Pediatric First Aid and CPR expires on 04/2026. LPA observed that the Licensee does have proof of Mandated Reporter AB 1207 compliant Child Care Training Certificate on file with expiration on 11/2026 www.mandatedreporterca.com At 11:15 am , LPA observed 4 staff members did not have complete immunization`s.. At 11:30 am six children\342\200\231s records were reviewed, including emergency information, and were observed to be complete. LPA created an LIC 857, Children\342\200\231s Record Review and LIC 859 for Staff Record Review , which documents staff and children\342\200\231s files reviewed during this inspection. Smoking is prohibited in a licensed Family Child Care Home. Per Licensee, no one smokes in the home. LPA did not observe the following items during the inspection: Infant Walkers, Johnny Jumpers, Saucer Chairs, and/or any other item that fall into these categories are not permitted in a family childcare facility. Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02- CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. Page 3 of 4 LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. LPA advised the licensee how to access forms, regulations and quarterly updates on the Child Care Licensing website at www.ccld.ca.gov . Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain childcare by connecting them to childcare providers and Resource and Referral Agencies (R&Rs) throughout California Based on the LPA\342\200\231s observations and records review, the following deficiencies listed on the attached LIC 809D (deficiency page) are being cited in accordance with California Code of Regulations Title 22. Deficiencies that are being cited need to be cleared to protect the children\342\200\231s health & safety. LPA discussed all the deficiencies cited during today\342\200\231s inspection with an assistant. Per Assistant , she understands all the deficiencies and plan of correction . A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
