What the inspector wrote

Licensing Program Analyst, T. Tran made an unannounced visit at the above licensed facility to conduct an investigation regarding the above complaint allegation. LPA met with Kayla De Leon, Site Supervisor. About 10:15AM, LPA and facility representative (FR) toured the facility and observed in the kitchen area under the cabinet sink, there was a rodent trapped in the trap. LPA and FR took photos for the record. About 10:55AM, maintenance staff arrival at the site to clean the kitchen area and removed the dead rodent. LPA interviewed staff and children and obtained some documents. Per FR, the facility had been taking action for rodents’ treatment. Record of Pest Control visited the site on 05/2/2022 and sealed 2 holds under the sink and set out traps for rodents. Therefore, based on the preponderance of evidence, the allegation of the facility had rodent is substantiated. A finding means that the complaint is substantiated, and the allegation is valid because the preponderance of the evidence standard has been met. Facility was cited type B deficiency. Please see Complaint Investigation Report LIC 9099D for deficiency cited. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the facility representatives, Michelle Harris and Kayla De Leon.

Based on the document reviewed, facility had providedreasonable information addressing the pest issue which poses an potential health and safety risk to children in care.
