What the inspector wrote

Licensing Program Analyst (LPA) Katrina Chicote conducted an unannounced case management inspection on 10/18/2022 at 1:15 PM to follow up on an incident that was reported to the Department on 10/04/2022 . Upon arrival, LPA met with Kayla De Leon , Site Supervisor, who provided LPA a tour of the facility inside and outside. Census was taken inside Classroom 3/4. On 10/04/2022 , an incident was reported to the department where a Police Officer came to the school and interviewed teacher and Site Supervisor in regards to a teacher allegedly hitting a child. The facility reported this incident to the Department within the required 24 hours and submitted a written report within seven days of call meeting Department's reporting requirements. During this inspection LPA interviewed Site Supervisor, staff (S1 and S2), and children (C1 and C2) in regards to incident and allegation made. LPA observed classroom, which is a large shared classroom between Class 3 and 4, where alleged incident took place. Site Supervisor informed LPA that Parent of child reported alleged incident to her on 09/15/2022 and she reported it to her Supervisor as well as discussed allegation with S1. Per Site Supervisor, Family Advocate reached out to parent multiple times for follow up but was unable to discuss incident further. Last contact made with Parent was on 09/20/2022 when Family Advocate finally made contact and stated that their child would no longer be attending. Interview conducted with staff and children did not provide any corroborating information in regards to incident. S1 states that child only attended for less than a week, Site Supervisor confirms this information. Based on the information obtained and LPA observation of the area, LPA determined there is not enough preponderance of evidence to substantiate the allegation. Therefore, there were no deficiencies cited on this date. The Notice of Site Visit (LIC 9213) \342\200\223 must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00. Exit interview conducted and report reviewed with, Kayla De Leon.
